Triamcinolone Acetonide (Dental Paste)

Indications

Triamcinolone Acetonide Dental Paste USP, 0.1% is indicated as an adjunctive therapy for the temporary relief of symptoms associated with inflammatory and ulcerative lesions of the oral mucosa, including those resulting from trauma.

Pharmacology

Like other topical corticosteroids, triamcinolone acetonide exhibits anti-inflammatory, antipruritic, and vasoconstrictive properties. The precise mechanism of its anti-inflammatory action is not fully understood. However, corticosteroids are believed to exert their effects through the induction of phospholipase A₂ inhibitory proteins, collectively known as lipocortins. These proteins are thought to regulate the biosynthesis of potent inflammatory mediators, such as prostaglandins and leukotrienes, by inhibiting the release of their common precursor, arachidonic acid. Arachidonic acid is normally released from membrane phospholipids by the enzyme phospholipase A₂.

Dosage Administration

Press a small dab (about 1/4 inch) to the lesion until a thin film develops. A larger quantity may be required for coverage of some lesions. For optimal results use only enough to coat the lesion with a thin film. Do not rub in. Attempting to spread this preparation may result in a granular, gritty sensation and cause it to crumble. After application, however, a smooth, slippery film develops. The preparation should be applied at bedtime to permit steroid contact with the lesion throughout the night. Depending on the severity of symptoms, it may be necessary to apply the preparation two or three times a day, preferably after meals. If significant repair or regeneration has not occurred in seven days, further investigation is advisable.

Contraindications

Triamcinolone Acetonide Dental Paste is contraindicated in patients with a known hypersensitivity to any component of the formulation. It is also contraindicated in the presence of fungal, viral, or bacterial infections of the mouth or throat.

Side Effects

The following local adverse reactions have been reported with corticosteroid-containing dental pastes: burning, itching, irritation, dryness, blistering or peeling not present before treatment, perioral dermatitis, allergic contact dermatitis, maceration of the oral mucosa, secondary infection, and atrophy of the oral mucosa.

Pregnancy & Lactation

Pregnancy Category C: Triamcinolone acetonide has been shown to cause birth defects in some animal studies. It is not known whether oral use of corticosteroids leads to significant amounts in breast milk. Exercise caution when prescribing corticosteroid-containing dental pastes to nursing women.

Precautions & Warnings
  • Local adverse reactions such as irritation may occur. If irritation develops, discontinue the paste and start appropriate therapy.
  • Allergic contact sensitization is usually recognized by delayed healing rather than acute flare-ups. Patch testing can help confirm the diagnosis.
  • If a mucosal infection is present or develops, use an appropriate antifungal or antibacterial agent. Discontinue the paste until the infection is controlled if a prompt response is not observed.
  • If oral tissue repair or regeneration is not significant within 7 days, further investigation into the cause of the lesion is advised.
  • Systemic absorption of topical corticosteroids may suppress the hypothalamic-pituitary-adrenal (HPA) axis, causing Cushing’s syndrome, hyperglycemia, glucosuria, or other steroid-related effects. Patients on long-term therapy should be monitored periodically for HPA suppression. If detected, reduce the dose or discontinue therapy. Recovery of HPA axis function is generally rapid after stopping treatment.
Special Populations

Pediatric Use: Safety and efficacy in children are unknown. Children may be more susceptible to HPA suppression and Cushing’s syndrome due to a higher surface area-to-body weight ratio. Use the minimum effective dose and monitor growth in chronic use.

Geriatric Use: Clinical data in patients over 65 are limited, but no significant differences in response have been reported compared to younger adults.
