Indications

Butamirate Citrate is indicated for the relief of dry (non-productive) cough, which may occur due to recent viral infections. It is also used to manage cough before and after surgical procedures, including bronchoscopy. Additionally, it is effective in treating acute cough of various causes, including whooping cough and cough associated with acute lower respiratory tract infections such as tracheitis, laryngitis, and bronchitis.

Pharmacology

Butamirate Citrate acts centrally on the cough center in the brain to suppress the cough reflex. It is a non-opioid, non-sedating agent and is not chemically or pharmacologically related to opium alkaloids. After oral administration, it is rapidly and completely absorbed. Peak plasma concentration is achieved within approximately 9 hours for sustained-release formulations. It is highly protein-bound, and its elimination half-life is around 13 hours. Its active metabolites also contribute to its antitussive (cough-suppressing) effect.

Dosage Administration

Use in adult:

  • Butamirate Citrate 50 mg tablet: 2-3 tablets daily.
  • Butamirate Citrate syrup: 15 ml 4 times daily.

Use in children & adolescents:
Butamirate Citrate 50 mg tablet:

  • Adolescent over 12 years old: 1-2 tablets daily.

Butamirate Citrate syrup:

  • Children (3-6 yrs): 5 ml 3 times daily.
  • Children (6-12 yrs): 10 ml 3 times daily.
  • Adolescent: 15 ml 3 times daily.

Butamirate Citrate pediatric drops:

  • Children (2 months- 1 yrs): 0.50 ml 4 times daily.
  • Children (1-3 yrs): 0.75 ml 4 times daily.
Interactions

Concurrent use of Butamirate Citrate with expectorants should be avoided.

Contraindications

This medication is contraindicated in patients with known hypersensitivity to Butamirate Citrate or any of its components.

Side Effects

Butamirate Citrate is generally well tolerated. Rare adverse effects may include rash, nausea, diarrhea, and dizziness, which usually resolve after dose reduction or discontinuation of therapy.

Pregnancy & Lactation

Butamirate Citrate should not be used during the first trimester of pregnancy. During the later stages of pregnancy, it may be used only if clearly indicated and with caution. Due to insufficient data regarding its excretion into breast milk, the benefits of use during breastfeeding should be carefully weighed against potential risks.

Precautions & Warnings

Since Butamirate Citrate suppresses the cough reflex, it should not be used with expectorants as it may cause mucus retention in the airways, increasing the risk of bronchospasm and respiratory infections. If cough persists for more than 7 days (or more than 3 days in children under 12 years), medical advice should be sought.

Overdose Effects

Accidental overdose may lead to symptoms such as drowsiness, nausea, vomiting, diarrhea, imbalance, and low blood pressure. Management includes standard emergency measures such as activated charcoal, saline laxatives, and supportive cardio-respiratory care.
