Calcipotriol is a non-steroidal antipsoriatic agent derived from vitamin D. It acts by binding to vitamin D receptors, similar to the active form of vitamin D3 (1,25(OH)₂D₃), helping regulate cell growth and differentiation. It promotes keratinocyte differentiation and inhibits excessive proliferation without causing cytotoxicity, thereby correcting abnormal skin changes in psoriasis and normalizing epidermal growth.
Dosage Administration
Calcipotriol cream & ointment should be applied topically to the affected area twice daily (i.e. in the morning and in the evening). Less frequent application may be indicated after the initial period of treatment. After satisfactory improvement has occurred, treatment should be discontinued. If recurrence takes place after discontinuation, the treatment may be reinstituted. Experience is lacking in the use of calcipotriol for periods longer than 1 year. The maximum recommended weekly dose of calcipotriol cream is 100 gm/week.
Contraindications
Calcipotriol cream and ointment are contraindicated in patients with known hypersensitivity to any of its components and in those with disorders of calcium metabolism.
Side Effects
Reported adverse effects include photosensitivity, skin discoloration, blistering eruptions, peeling of the skin, contact dermatitis, and allergic reactions.
Pregnancy & Lactation
The safety of calcipotriol during pregnancy has not been established. It should be used only if the benefits outweigh potential risks. It is unknown whether calcipotriol is excreted in breast milk; therefore, it should be used cautiously during lactation. Avoid application on the breast area during breastfeeding to prevent accidental ingestion by the infant.
Precautions & Warnings
Calcipotriol cream and ointment are not recommended for patients with generalized pustular psoriasis, guttate psoriasis, or erythrodermic psoriasis. It should also not be applied to the face.
Special Populations
Calcipotriol cream and ointment are not recommended for use in children due to insufficient clinical experience.
