Oxyphenonium Bromide

Indications

Oxyphenonium is indicated for the management of gastrointestinal spasm and hypermotility, cardiospasm, esophagospasm, gastroduodenitis, spastic constipation, pylorospasm, peptic ulcer, and spasm associated with carcinoma. It is also used for pain and spasm of the biliary tract as well as pain and spasm of the urinary tract.

Pharmacology

Oxyphenonium is an anticholinergic medicine that reduces the effect of acetylcholine, a chemical released from nerves that stimulates muscle activity. It works by blocking acetylcholine receptors on smooth muscles, thereby reducing muscle contractions. It also has a direct relaxing effect on smooth muscles. Oxyphenonium is used to treat or prevent spasms of the gastrointestinal tract, especially in conditions such as irritable bowel syndrome. In addition, it reduces gastrointestinal motility, decreases gastric acid secretion, and helps control excessive secretions in the pharynx, trachea, and bronchi.

Dosage Administration

Average adult dosage is 10 mg (2 tablets) 4 times daily for several days. Dosage may be reduced depending on patient's response or as directed by the physician.

Interactions

Reduced gastric motility caused by Oxyphenonium may interfere with the absorption of other medicines. Its effects may be increased when used together with other antimuscarinic drugs, including amantadine, certain antihistamines, phenothiazines, and tricyclic antidepressants (TCA).

Contraindications

Oxyphenonium is contraindicated in patients with glaucoma, obstructive uropathy (such as bladder neck obstruction due to prostatic hypertrophy), obstructive gastrointestinal diseases (such as achalasia, paralytic ileus, pyloroduodenal stenosis), intestinal atony in elderly or debilitated patients, unstable cardiovascular status in acute hemorrhage, severe ulcerative colitis, toxic megacolon associated with ulcerative colitis, myasthenia gravis, and hypersensitivity to the drug.

Side Effects

Common side effects include dry mouth, headache, palpitations, and related symptoms.

Pregnancy & Lactation

Animal studies have shown adverse effects on the fetus (such as teratogenic or embryocidal effects), and there are no adequate controlled studies in pregnant women. This medicine should be used only when the potential benefit outweighs the possible risk to the fetus.

Precautions & Warnings

Use with caution in children, elderly patients, benign prostatic hyperplasia, acute myocardial infarction, cardiac failure, hypertension, thyrotoxicosis, pregnancy, lactation, fever, and angle-closure glaucoma.

Overdose Effects

Treatment of overdose is similar to the management of atropine overdose.

Store below 30°C temperature, away from light and moisture. Keep out of reach of children.
