Pyrantel Pamoate acts as a depolarizing neuromuscular blocking agent, causing sustained contraction followed by paralysis of helminths. This results in loss of attachment of worms from the intestinal wall, allowing them to be expelled naturally through the gastrointestinal tract. Since pyrantel is poorly absorbed from the gastrointestinal tract, systemic exposure in the host is minimal, resulting in low toxicity. In cases of heavy worm burden, spastic paralysis of worms may lead to transient intestinal obstruction due to mass expulsion. Worms are usually passed in normal stool, sometimes with diarrhea, abdominal straining, or occasional vomiting.
Dosage Administration
Pyrantel Pamoate is given orally at anytime without regard to ingestion of food or beverages. A single dose of 11 mg/kg body weight, to a maximum of 1 g, should be given to treat infestations caused by the parasites mentioned above. In the case of Pinworm, it is often wise to repeat the dose after an interval of 2 weeks.
Contraindications
Known hypersensitivity to Pyrantel Pamoate or any component of the formulation.
Side Effects
Pyrantel Pamoate is generally well tolerated at recommended doses. At higher doses, it may cause gastrointestinal disturbances such as loss of appetite, nausea, vomiting, and diarrhea. Rare adverse effects include headache, dizziness, and skin rash.
Pregnancy & Lactation
Pregnancy: Safety in pregnant women has not been established; therefore, use is generally contraindicated. Pyrantel Pamoate is not recommended for children below 1 year of age.
Precautions & Warnings
Pyrantel Pamoate should not be used concurrently with Piperazine due to antagonistic effects. Keep out of reach of children.
Special Populations
Pediatric Use: Safety and efficacy are not established in children under 2 years. Use only if benefits outweigh risks.
Hepatic Impairment: Use with caution in patients with liver dysfunction.
