Racepinephrine

Racepinephrine is an alpha- and beta-adrenergic receptor agonist. It works by stimulating these receptors to cause relaxation of bronchial smooth muscles and widening of the airways. This reduces bronchospasm in asthma patients and helps improve breathing.
Dosage Administration
Adults and children 4 years of age and over
- Jet Nebulizer: Nebulisation for fifteen minutes every 3 to 4 hours. One 0.5 ml ampoule should be diluted to a volume of 3 to 5 ml with sterile 0.9% sodium chloride solution.
- Hand-held Bulb Nebulizer: 1 to 3 inhalations not more often than every 3 hours. The racepinephrine inhalation solution does not require dilution. Should not use more than 12 inhalations in 24 hours.
Children under 4 years of age: Use according to the physician's direction.
Interactions
Concomitant use of racepinephrine with other adrenergic agents may produce additive sympathomimetic effects. Use with monoamine oxidase (MAO) inhibitors may result in hypertensive crisis. Beta-blockers may reduce or negate its therapeutic effect. Tricyclic antidepressants may enhance the response to epinephrine.
Side Effects
Racepinephrine may cause headache, loss of appetite, nausea, vomiting, nervousness, tremors, insomnia, fear, agitation, anxiety, restlessness, dizziness, impaired memory, hallucinations, and psychosis. Serious allergic reactions may occur, including rash, urticaria, itching, difficulty in breathing, and swelling of the face, lips, mouth, or tongue. Cardiovascular effects such as chest pain, tachycardia, hypertension, and arrhythmias may also occur. In patients with coronary artery disease or hypertension, symptoms may worsen
Pregnancy & Lactation
Racemic epinephrine is a 1:1 mixture of dextrorotatory and levorotatory isomers of epinephrine. It is classified as FDA pregnancy category C. It should be used during pregnancy only when no safer alternative exists and the potential benefit outweighs the risk. Epinephrine is excreted in human breast milk. Due to the potential risk of serious adverse effects in nursing infants, a decision should be made to discontinue breastfeeding or discontinue the drug, considering maternal benefit.
Precautions & Warnings
Racepinephrine should be used with caution in patients with cardiac disease (angina, tachycardia, myocardial infarction, hypertension), hyperthyroidism, diabetes, or cerebral arteriosclerosis. Excessive use may lead to tolerance or paradoxical bronchospasm. If symptoms do not improve within 20 minutes or worsen, further use is not recommended. The solution should not be used if it appears brown, cloudy, pink, or contains any precipitate.
Overdose Effects
Excessive use may cause nervousness, tachycardia, and adverse cardiovascular effects. In case of overdose, immediate medical attention is required.
