Indications

Ramosetron Hydrochloride is indicated for diarrhea-predominant irritable bowel syndrome (IBS-D).

Pharmacology

Ramosetron Hydrochloride is a selective serotonin (5-HT3) receptor antagonist. Serotonin is an important neurotransmitter involved in gastrointestinal motility, secretion, and sensation. Stress can increase serotonin release and activate 5-HT3 receptors in the enteric nervous system, leading to increased intestinal transit and diarrhea. Increased intracolonic pressure also triggers serotonin release, which binds to 5-HT3 receptors on afferent nerve terminals and transmits pain signals to the central nervous system. Ramosetron Hydrochloride selectively blocks 5-HT3 receptors, thereby reducing accelerated intestinal transit and improving abnormal bowel movements. It also reduces visceral hypersensitivity by inhibiting intestinal pain transmission, helping to relieve abdominal pain. Additionally, serotonin released from enterochromaffin cells (for example, due to cytotoxic drugs like cisplatin) stimulates vagal afferent nerves via 5-HT3 receptors and triggers vomiting through the vomiting center. Ramosetron exerts its antiemetic effect by blocking these receptors.

Dosage Administration

Male patients with diarrhea-predominant irritable bowel syndrome: The usual dose for male adults is 5 mcg of Ramosetron Hydrochloride, administered once daily orally. The dose may be adjusted according to the patients' clinical condition. The daily dose should not exceed 10 mcg.

Female patients with diarrhea-predominant irritable bowel syndrome: The usual dose for female adults is 2.5 mcg of Ramosetron Hydrochloride, administered once daily orally. The dose may be increased for patients who do not adequately respond to the initial dose. The daily dose should not exceed 5 mcg.

Interactions

Ramosetron Hydrochloride may interact with antidepressants (such as fluvoxamine), diuretics (e.g., bumetanide), antipsychotics, tricyclic antidepressants, anticholinergic drugs, opioid analgesics, and phenothiazine derivatives.

Contraindications

Hypersensitivity to the active substance or any of its components.

Side Effects

The most commonly reported adverse effects include constipation and hard stools. Other important side effects may include facial pallor, cold sweating, dizziness on standing, sudden abdominal pain, bloody stools, and fever.

Pregnancy & Lactation

Ramosetron Hydrochloride should be used during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Caution is advised when administering to breastfeeding mothers.

Precautions & Warnings

Use with caution in patients with constipation, as it may worsen the condition. Dose adjustment or alternative therapy may be required in some cases.

Use with caution in patients with liver disease due to increased risk of adverse effects. Regular monitoring of liver function tests is recommended, and dose adjustment or alternative therapy may be necessary based on clinical condition.
