Fosravuconazole L-lysine ethanolate is a prodrug of ravuconazole, a novel triazole antifungal agent with broad and potent antifungal activity. It exerts its antifungal effect by inhibiting the biosynthesis of ergosterol, an essential component of the fungal cell membrane. Disruption of ergosterol synthesis leads to impaired cell membrane integrity and fungal cell death. Ravuconazole is structurally related to fluconazole and voriconazole, but contains a thiazole ring instead of a second triazole group. It is primarily used in the treatment of onychomycosis. It demonstrates strong activity against a wide range of fungi, including Aspergillus species (such as A. fumigatus and A. flavus), Candida species, Cryptococcus neoformans, and other yeasts, including fluconazole-resistant strains.
Dosage Administration
The usual adult dosage is 1 capsule (100 mg as Ravuconazole) administered orally once daily for a period of 12 weeks.
Interactions
The therapeutic effect of ravuconazole may be increased when used with amiodarone, amlodipine, bepridil, cinnarizine, clevidipine, and carvedilol. The effect of acenocoumarol may also be enhanced when combined with ravuconazole. Drugs such as cimetidine, calcium carbonate, almasilate, aluminium phosphate, aluminium hydroxide, and asenapine may reduce the absorption of ravuconazole, leading to decreased serum levels and reduced efficacy.
Contraindications
Contraindicated in patients with known hypersensitivity to ravuconazole or any of its components, especially those with a history of generalized allergic reactions.
Side Effects
The most commonly reported adverse effects include abdominal discomfort, constipation, and angular cheilitis. If these occur, medical advice should be sought.
Rare but serious symptoms may include:
- Loss of appetite, nausea, jaundice (liver dysfunction)
- Red or oval rash with fever and joint pain (erythema multiforme)
If any of these serious symptoms appear, the medication should be discontinued immediately and a physician consulted.
Pregnancy & Lactation
Individuals capable of becoming pregnant should use effective contraception during treatment and for at least 3 months after stopping this medicine.
Precautions & Warnings
Lesioned nails should be trimmed using a file or nail clipper as needed. Individuals capable of pregnancy must use reliable birth control during treatment and continue it for at least 3 months after discontinuation of the medicine.
