Indications

Celiprolol Hydrochloride is a beta-blocker that reduces heart rate and lowers blood pressure. It is used in the management of high blood pressure (hypertension).

Pharmacology

Celiprolol is a cardioselective beta-1 adrenoceptor antagonist with partial beta-2 agonist activity. Its beta-2 agonist effect contributes to mild vasodilation. It effectively lowers blood pressure in hypertensive patients both at rest and during physical activity. The influence on heart rate and cardiac output depends on the baseline sympathetic tone. During stress conditions such as exercise, it reduces chronotropic and inotropic responses to sympathetic stimulation, while at rest it causes minimal impairment of cardiac function.

Dosage Administration

Adults including the elderly: the recommended starting dose is 200 mg of Celiprolol Hydrochloride once a day. If needed after 2 to 4 weeks, your doctor will increase your dose to 400 mg once a day. The maximum dose is 400 mg once a day. If you have the impression that the effect of Celiprolol Hydrochloride tablets is too strong or too weak, talk to your doctor or pharmacist.

Adults with liver or kidney problems: if you have kidney problems, your doctor may give you a lower dose than stated above and increase it as needed. A reduction in dosage by half may be appropriate in patients with moderate impairment of kidney function (creatinine clearance in the range of 15-40 ml per minute). Celiprolol Hydrochloride is not recommended for patients with severely impaired kidney function (creatinine clearance less than 15 ml per minute). Use of low doses of Celiprolol Hydrochloride is recommended in patients with liver problems.

Administration

Always take this medicine exactly as your doctor has told you. Swallow the tablets whole with a glass of water. Take the tablets preferably first thing in the morning, one hour before food or two hours after food. To help you remember to take your medicine, try to get into the habit of taking it at the same time each day, preferably first thing in the morning.

Contraindications

Do not take Celiprolol Hydrochloride if you:

  • are allergic to celiprolol hydrochloride or any component of this medicine. Signs of an allergic reaction include rash, difficulty swallowing or breathing, swelling of the face, lips, throat or tongue
  • have had a previous allergic reaction to beta-blockers (such as propranolol, sotalol, timolol, etc.)
  • have a very slow or irregular heartbeat (bradycardia) or low blood pressure (hypotension)
  • have uncontrolled heart failure or abnormal heart rhythm, breathing difficulty, or swelling of ankles
  • have heart block or other significant heart disorders, or suffer from hardened arteries
  • have severe kidney impairment
  • have or had asthma, wheezing, or severe respiratory problems
  • have an untreated tumour of the adrenal gland (phaeochromocytoma)
  • have metabolic acidosis (increased acid levels in the blood)
  • are taking theophylline (used for asthma)
  • have advanced peripheral arterial disease or Raynaud’s syndrome
Side Effects

Serious side effects: Allergic reactions such as rash, difficulty swallowing or breathing, and swelling of the face, lips, throat, or tongue. Other symptoms may include flu-like illness, joint pain or swelling, sensitivity to sunlight, hair loss, abdominal pain, and anemia. These may indicate a condition such as Systemic Lupus Erythematosus (SLE). These reactions are serious and require immediate medical attention or hospitalization.

Common side effects (may affect up to 1 in 10 people): Dizziness, light-headedness, weakness, or fainting, especially when standing up quickly, which may indicate low blood pressure. Skin rash and itching may also occur.

Pregnancy & Lactation

If you are pregnant, breastfeeding, planning to become pregnant, or think you might be pregnant, consult your doctor or pharmacist before taking this medicine. Your doctor will determine whether Celiprolol Hydrochloride is suitable for you. If taken during pregnancy, inform your healthcare provider. In newborns, beta-blockers may persist for several days and can cause effects such as slow heart rate, heart failure, or breathing difficulties. Therefore, careful monitoring of the newborn is recommended during the first 3 to 5 days of life. If you become pregnant while taking this medicine, contact your doctor immediately. Breastfeeding is not recommended while taking Celiprolol Hydrochloride. If you are breastfeeding or planning to do so, seek medical advice before use.

Precautions & Warnings

Take special care and consult your doctor before using Celiprolol Hydrochloride if you:

  • have liver or kidney problems
  • have Prinzmetal’s angina (chest pain at rest)
  • have poor blood circulation (e.g., Raynaud’s disease or Buerger’s disease); symptoms of Raynaud’s include fingers and toes changing color in cold and tingling or pain when warming up; your doctor will monitor you closely
  • have reduced blood flow in the coronary arteries
  • have a history of psoriasis; treatment may need to be stopped if symptoms worsen
  • have asthma or chronic respiratory diseases such as bronchitis or emphysema
  • have thyroid disorders, particularly hyperthyroidism (thyrotoxicosis), which may be masked by beta-blockers
  • have a history of severe allergies, including anaphylactic reactions
  • are receiving treatment for allergies (e.g., after insect stings); your doctor may temporarily discontinue this medicine
  • have diabetes; symptoms of low blood pressure or hypoglycemia may be masked by beta-blockers
  • have heart failure that is currently controlled with treatment; use should be under strict medical supervision
  • have a slow or irregular heartbeat
Special Populations

Use in children: Celiprolol Hydrochloride is not recommended for use in children.
Use in elderly: Dose adjustment may be required based on clinical condition.

Overdose Effects

Overdose may present with symptoms such as slow heart rate, low blood pressure, dizziness, weakness, breathing difficulty, wheezing, or cardiac complications. In such cases, seek immediate medical attention. Take the medicine packaging or leaflet with you to help healthcare providers identify the drug taken.
