Cetirizine is an antihistamine that acts as a selective histamine-1 (H1) receptor antagonist. Its therapeutic effect is achieved by blocking H1 histamine receptors. Antihistaminic activity of cetirizine has been demonstrated in both animal and human studies. In vivo and ex vivo studies indicate minimal anticholinergic and antiserotonergic effects. In vitro studies show that cetirizine has selective affinity for H1 receptors with negligible binding to other receptor types.
In healthy individuals, administration of one drop of cetirizine 0.24% ophthalmic solution in both eyes resulted in a mean peak plasma concentration (Cmax) of 1.7 ng/mL after a single dose and 3.1 ng/mL after twice-daily use for one week. The average terminal half-life of cetirizine was approximately 8.6 hours after a single dose and 8.2 hours following twice-daily dosing for one week.
Dosage Administration
Adults and children (age 2 years and above): The recommended dosage is to instill one drop in the affected eye(s) twice daily (approximately 8 hours apart).
Side Effects
The most commonly reported adverse effects include eye redness (ocular hyperemia), pain at the site of instillation, and a temporary decrease in visual clarity.
Pregnancy & Lactation
Pregnancy: There are no adequate and well-controlled studies of Cetirizine 0.24% ophthalmic solution in pregnant women. It should be used during pregnancy only if the expected benefit outweighs the potential risk to the fetus.
Lactation: It is unknown whether cetirizine, after ocular administration, is excreted into human breast milk in measurable amounts. There is insufficient data regarding its effects on breastfed infants or milk production. The benefits of breastfeeding and the mother’s need for treatment should be considered along with any potential risk to the infant.
Precautions & Warnings
Avoid touching the dropper tip to the eyelids or surrounding areas to prevent contamination. Keep the bottle tightly closed when not in use. Patients should not wear contact lenses if the eyes are red. Contact lenses should be removed before applying the drops and may be reinserted after 10 minutes.
Special Populations
Pediatric use: Safety and effectiveness have been established in children aged 2 years and above.
Geriatric use: No significant differences in safety or effectiveness have been observed between elderly and younger patients.
