Chlorhexidine: Chlorhexidine is an antiseptic and disinfectant effective against a wide range of Gram-positive and Gram-negative bacteria, as well as some viruses and fungi. It is not effective against bacterial spores at room temperature, and acid-fast bacteria are inhibited but not completely destroyed. It shows greater activity against Gram-positive organisms, while some strains of Pseudomonas and Proteus are relatively less sensitive. Chlorhexidine works best at neutral or slightly acidic pH, and its activity may be reduced in the presence of blood or organic matter.
Cetrimide: Cetrimide is a quaternary ammonium compound with disinfectant properties and acts as a cationic surfactant. It has emulsifying and detergent actions along with bactericidal effects against both Gram-positive and Gram-negative organisms, although higher concentrations are needed for Gram-negative bacteria. Its combined cleansing and antibacterial properties make it useful for cleaning contaminated or infected wounds. However, it is less effective against bacterial spores, acid-fast bacteria, viruses, and fungi.
Dosage Administration
Rinse the area to be cleaned with water, apply the minimum amount of irrigation necessary to cover the wound area and wash gently. Rinse again thoroughly. Apply to wound as necessary. Discard remaining solution after use.
Paediatric Use: Use with care in neonates, particularly in premature infants. Chlorhexidine may cause irritation or chemical burns.
Contraindications
Avoid use in patients with known hypersensitivity to chlorhexidine or cetrimide. It should not be used for irrigation of the brain, meninges, eyes, or in cases of a perforated eardrum.
Side Effects
Skin irritation and allergic (hypersensitivity) reactions associated with chlorhexidine have been reported. If such reactions occur, discontinue use. Cetrimide may delay the wound healing process in some cases.
Pregnancy & Lactation
Pregnancy: There is insufficient data regarding the use of chlorhexidine and cetrimide in pregnant women. However, systemic risk is expected to be very low due to minimal absorption through the skin.
Breastfeeding: It is not known whether chlorhexidine and cetrimide are excreted in breast milk. Due to poor absorption, significant transfer is unlikely. As a precaution, wash the nipple area thoroughly with water before breastfeeding if the product has been applied.
Precautions & Warnings
For external use only. Not suitable for injection. Intended for irrigation use only. This preparation is not isotonic and may cause hemolysis. Cetrimide may prolong wound healing. Its use should be limited to infection control and wound cleansing rather than routine wound management.
Overdose Effects
Accidental ingestion is unlikely to produce systemic effects due to poor absorption. However, ingestion of large amounts may irritate the gastrointestinal tract and cause gastritis. Gastric lavage and symptomatic treatment may be required. If swallowed, rinse the mouth, drink plenty of water or milk, and seek medical advice. In case of overdose, contact a healthcare provider or poison control center immediately.
