Cilostazol is indicated for the improvement of symptoms of intermittent claudication, as evidenced by an increased walking distance. Intermittent claudication refers to muscle discomfort such as pain, cramping, numbness, or fatigue that occurs during mild physical activity.

Cilostazol is a quinolinone derivative that selectively inhibits phosphodiesterase III (PDE III), leading to suppression of cAMP breakdown. This results in increased levels of cAMP in platelets and vascular tissues, thereby inhibiting platelet aggregation and promoting vasodilation.
Dosage Administration
The recommended dosage of Cilostazol is 100 mg bid, taken at least half an hour before or two hours after breakfast and dinner. A dose of 50 mg bid should be considered during coadministration of Fluconazole, Ketoconazole, Itraconazole, Erythromycin, Clarithromycin, Fluvoxamine, Fluoxetine, Nefazodone, Sertraline and Diltiazem.
Pediatric use: The safety and effectiveness of Cilostazol in pediatric patients have not been established.
Interactions
Pharmacokinetic studies have shown that omeprazole and erythromycin can significantly increase the systemic exposure of cilostazol and its metabolites. Increased plasma levels of cilostazol have also been observed in patients receiving diltiazem concurrently.
Contraindications
Cilostazol is contraindicated in patients with congestive heart failure of any severity. It is also contraindicated in individuals with known or suspected hypersensitivity to cilostazol or any of its components.
Side Effects
Common side effects include headache, diarrhea, vomiting, leg cramps, and skin rash. Less common adverse effects include loss of appetite (anorexia) and edema.
Pregnancy & Lactation
There are no adequate and well-controlled studies in pregnant women. Animal studies have shown that cilostazol may be excreted into breast milk. Due to potential risks to the infant, a decision should be made whether to discontinue breastfeeding or stop the medication.
Precautions & Warnings
Cilostazol should be used cautiously in patients with any degree of heart failure. There is limited information regarding the safety and effectiveness of using cilostazol together with clopidogrel.
