Indications

This combination is indicated for the management of:

  • Internal and external hemorrhoids
  • Postpartum hemorrhoids
  • Anal itching, perianal eczema, anal fissures, and proctitis
  • Relief of pain and discomfort following hemorrhoid surgery
  • Use medicines only as advised by a registered physician
Composition

Each gram of ointment contains:

  • Cinchocaine Hydrochloride BP 5 mg
  • Hydrocortisone BP 5 mg
  • Framycetin Sulphate BP 10.5 mg
  • Esculin Ph. grade 10 mg

Each suppository contains:

  • Cinchocaine Hydrochloride BP 5 mg
  • Hydrocortisone BP 5 mg
  • Framycetin Sulphate BP 10 mg
  • Esculin Ph. grade 10 mg
Pharmacology

Cinchocaine Hydrochloride acts as a local anesthetic that helps relieve severe pain associated with conditions such as hemorrhoids, anal fissures, and perianal hematoma. Hydrocortisone, a corticosteroid, reduces inflammation, swelling, and itching, and also helps decrease mucus discharge. Framycetin Sulphate, a broad-spectrum antibiotic, works against a wide range of bacteria to treat or prevent infections in anorectal lesions. Esculin provides a protective effect on the skin and supports vascular integrity.

Dosage Administration

Ointment: Apply the ointment in a small quantity with the finger, on the painful or pruritic area, morning and evening and after each stool. For deep application, insert the cannula or applicator to full extent and squeeze tube gently from lower end whilst withdrawing.

Suppository: A suppository is inserted morning and evening, and after each stool.

Contraindications

This preparation is contraindicated in patients with known hypersensitivity to any of its four components. Corticosteroids have demonstrated teratogenic effects in animal studies following topical application. As these agents can be absorbed through the skin, a potential risk to the fetus cannot be excluded; therefore, this product should not be used during pregnancy. Topical corticosteroids are also contraindicated in conditions such as herpes simplex, vaccinia, varicella, or tuberculous infections of the anal region.

Side Effects

Prolonged and continuous use of topical corticosteroids should be avoided, as it may lead to skin atrophy characterized by thinning, reduced elasticity, dilation of superficial blood vessels, telangiectasia, and bruising. These effects are more likely when occlusive dressings are used. Systemic absorption may occur, particularly when large amounts are applied, when used over extensive or damaged skin, or when potent corticosteroids are used with occlusive techniques. This may result in suppression of the hypothalamic–pituitary–adrenal axis and reduced adrenal gland function. These effects are more pronounced in children and may lead to growth retardation and Cushingoid features. Rare cases of benign intracranial hypertension have been reported.

Pregnancy & Lactation

The safety of topical corticosteroids during pregnancy has not been fully established. Therefore, their use should be limited and avoided over large areas, in high amounts, or for prolonged periods during pregnancy.

Precautions & Warnings

Treatment should be discontinued if signs of sensitivity occur. Appropriate management of underlying infections, allergies, or other contributing conditions should not be overlooked. Although uncommon, prolonged use may lead to systemic corticosteroid effects, and this possibility should be considered. Patients should inform healthcare providers about prior use of hydrocortisone.
