Simethicone

Indications

Simethicone is indicated for the relief of flatulence, abdominal distension, fullness, gas, and colicky pain caused by excessive intestinal gas. It is an effective antiflatulent used to reduce discomfort associated with excess gas in the gastrointestinal tract, which may result from swallowing air or consuming certain foods.

Simethicone drops are commonly used in infants and act in both the stomach and intestines, helping gas to be released more easily through belching or flatulence.

It is also used as an adjunct in bowel preparation with polyethylene glycol to reduce gas-related discomfort prior to diagnostic procedures. Additionally, it has been used in cases of accidental ingestion of foaming detergents as an antifoaming agent.

Pharmacology

Simethicone is an antiflatulent used to relieve symptoms of gas such as abdominal bloating, pressure, fullness, and discomfort in the gastrointestinal tract. Its clinical effect is based on its antifoaming action. It works by reducing the surface tension of gas bubbles in the stomach and intestines, allowing smaller bubbles to combine into larger ones. This facilitates easier expulsion of gas through belching or flatulence.

Simethicone also helps reduce postoperative gas discomfort and is used before diagnostic procedures such as gastroscopy to improve visualization and before intestinal radiography to reduce gas shadows.

Dosage Administration

Take after meals and at bedtime. Can be given with infant's feeds. Shake the bottle well before each use.

Children less than 2 years of age: 20 mg (0.3 ml Simethicone Paediatric Drops) 4 times daily up to 240 mg/day (3.6 ml Simethicone Paediatric Drops).

Children 2-12 years of age: 40 mg (0.6 ml Simethicone Paediatric Drops) 4 times daily.

Adults:

  • 40-125 mg (0.6 ml-1.9 ml Simethicone Paediatric Drops) 4 times daily, up to 500 mg/day (7.5 ml Simethicone Paediatric Drops) or
  • 1-3 Simethicone chewable tablets; 4 times daily, up to 500 mg/day (12 Simethicone chewable tablets).
Interactions

There is no evidence that Simethicone alters the effect of other drugs. However, its defoaming action may be reduced when taken with antacids such as aluminium hydroxide and magnesium carbonate, as these may absorb the silicone component.

Side Effects

Simethicone is physiologically inert, and no adverse effects have been reported following oral administration.

Pregnancy & Lactation

Pregnancy: No evidence is available to suggest harmful effects during pregnancy.

Lactation: Excretion of Simethicone into breast milk has not been established and is considered highly unlikely.

Precautions & Warnings

Do not exceed 12 doses per day unless directed and supervised by a physician.
