Indications

Eye/Ear/Nasal Solution: This is indicated for dry nasal membranes, including dryness caused by cold or allergy medications. It helps moisturize dry nasal passages due to dry climates or airplane travel. It also helps dissolve thick mucus, clears nasal passages after surgery, and is used to cleanse wounds, body cavities, and medical equipment such as bandages, catheters, and drainage tubes. It may also be used as a diluent for other medications used in irrigation (e.g., bacitracin, polymyxin).

Intravenous Solution: Used in the management of metabolic alkalosis, gastroenteritis, and during or after surgery. It may also be used as a priming fluid in hemodialysis. Sodium Chloride 0.9% is also commonly used as a diluent for intravenous drug preparations and for sterile irrigation.

Nebulizer Solution: Used to help remove thick mucus (phlegm) from the lungs in conditions such as bronchiolitis, asthma, COPD, atelectasis, cystic fibrosis, and bronchiectasis.

Pharmacology

Nasal Drop: Sodium Chloride 0.9% is a non-drug isotonic saline solution that moisturizes nasal membranes. It softens thick or crusted mucus, helping its removal and making breathing easier. Excess sodium is mainly excreted via the kidneys, with small amounts lost in feces and sweat.

Nasal Spray: Sodium Chloride 0.9% helps moisten the inside of the nose by dissolving and softening thick mucus, making it easier to remove and improving breathing. In infants and young children who cannot blow their noses, it helps facilitate mucus removal.

Sterile Nebulizer Solution: It draws water from the airway walls into the mucus, making it thinner and less sticky. This helps in easier expectoration of phlegm from the lungs.

Dosage Administration

Nasal drop
Nasal Congestion: In infants, 1-2 drops of Sodium Chloride nasal wash in each nostril before feeds will help relieve congestion and allow more effective suckling.

Ear Wax Removal: Allow the Sodium Chloride nasal wash to warm to room temperature before use. It is important to put the drops into the ear carefully. Lie down on one side and fill the ear canal with the drops and wait for a few minutes before standing up. Age: from 6 months onwards. Put 3 to 4 drops into the affected ear(s) 3 to 4 times a day for 3 to 5 days.

Nasal Spray
Newborn to 12 months: 1 spray into each nostril for 2-3 times or as directed by the physician.
Children over 12 months & Adults: 1 to 2 sprays into each nostril for 2-3 times or as directed by the physician.

For eye use: 1 or 2 drops in each eye when needed.

For ear use: 3 to 4 drops into the affected ear(s) 3 to 4 times a day for 3 to 5 days.

Intravenous Solution: The concentration and dosage of Sodium Chloride solution for intravenous use is determined by several factors including age, weight and clinical condition of the patient. Usually, the adult dose is about 1000 ml of 0.9% injection.

Nebulizer Solution: 4 ml nebulized for 20 minutes, 4 times daily (for all ages). The exact nebulization duration & volume should be as determined by the Physician.

How to use the Nasal Spray

Shake the bottle gently and remove the dust cover.
Hold the spray with your forefinger and middle finger on either side of the nozzle and your thumb underneath the bottle. Press down until a fine spray appears. If using for the first time or if you have not used it for a week or more, press the nasal applicator several times until a fine moist comes out from the container.

Gently blow the nose to clear the nostrils.
Close one nostril and carefully insert the nasal applicator into the open nostril. Tilt your head forward slightly and keep the spray upright. Breathe in through your nose and while breathing in, press the white-collar of nasal applicator firmly down once to release a spray.

Breathe out through your mouth.
Repeat the above steps in the same/other nostril for consecutive doses.

Cleaning: The nasal spray should be cleaned at least once a week. The procedures are as follows—
Remove the dust cover.
Gently pull off the nasal applicator.
Wash the applicator and dust cover in warm water.
Shake off the excess water and leave to dry in a normal place. Avoid to apply additional heat.
Gently push the applicator back on the top of the bottle and re-fix the dust cover.

How to use Intravenous Solution

Check infusion set and infusion solution prior to use.
Pull moderately to tear off the protective cover of the Eurocap.
Hold lightly the Eurocap but not the bottle body.
Open the flow regulator fully and hold the giving set on the top white area, but not the membrane venting region.
Insert the spike of the administration set to the Eurocap and fit the connector of the administration set firmly to the needle.
Gradually allow the fluid to flow down to the needletip and close.
Remove the protective cover of the needle.
Locate the venipuncture site and clean the site with an antiseptic solution, and then insert the needle.
Securely tape the puncture site.
Securely tape the wings and tubing.
Start infusion while adjusting drip speed.

How to use Nebulizer Solution

Step 1: Twist off the top of the ampoule. Be careful to hold the ampoule upright.
Step 2: Squeeze the desired amount of the nebulizer solution into the nebulizer chamber.

Contraindications

Do not share this medication with others, as it may increase the risk of infection.

Side Effects

Eye/Ear/Nasal: No significant side effects are expected. However, some people may experience mild stinging, sneezing, increased nasal discharge, or a salty taste.

Intravenous: Possible reactions related to the solution or administration technique include fever, infection at the injection site, venous thrombosis or phlebitis, extravasation, and hypervolemia. If any adverse reaction occurs, the infusion should be stopped immediately, the patient evaluated, and appropriate treatment given. The remaining fluid should be saved for examination if needed.

Pregnancy & Lactation

Eye/Ear/Nasal: Inform your doctor if you are pregnant before using this medicine. It is unknown whether it passes into breast milk. Consult your doctor before breastfeeding.

Intravenous: Pregnancy Category C. Animal studies have not been conducted. It is unknown whether Sodium Chloride 0.9% can cause fetal harm or affect fertility. It should be used during pregnancy only if clearly needed.

Precautions & Warnings

Eye/Ear/Nasal: Inform your doctor about medical history such as heart disease, lung disease, kidney problems, low potassium, high sodium levels, and allergies.

Intravenous: Serum electrolyte levels should be monitored carefully. Use with caution in patients with heart failure, pulmonary or peripheral edema, renal impairment, or pre-eclampsia. Extra caution is required in infants and elderly patients. Excess use may lead to hypokalemia. Infusion should be stopped immediately if chills occur. Do not use if the solution is cloudy, contains particles, or is expired.

Overdose Effects

No adverse reports of overdose have been observed with Sodium Chloride 0.9% nasal wash.

Keep out of reach of children. Store at room temperature (15°C–30°C), away from light and moisture.
