Clobetasone Butyrate preparations are indicated for the treatment of various forms of eczema and dermatitis, including atopic eczema, photodermatitis, otitis externa, primary irritant and allergic dermatitis (including napkin rash), intertrigo, prurigo nodularis, seborrhoeic dermatitis, and reactions to insect bites. It may also be used as maintenance therapy between courses of more potent topical corticosteroids.

Clobetasone Butyrate is a topically active corticosteroid that offers a favorable balance between efficacy and safety. It is more effective in managing eczema compared to 1% hydrocortisone and other less potent synthetic corticosteroids commonly used. It has minimal impact on the hypothalamic–pituitary–adrenal axis. Although all topical corticosteroids may cause skin atrophy when used excessively, studies in both animals and humans suggest that Clobetasone Butyrate causes less thinning of the epidermis compared to many other topical steroids.
Dosage Administration
Apply to the affected area up to four times a day until improvement occurs, when the frequency of application may be reduced.
Interactions
No potentially harmful or beneficial drug interactions have been reported with Clobetasone Butyrate.
Contraindications
This preparation is contraindicated in skin conditions caused by viral (e.g., herpes simplex, chickenpox), fungal (e.g., candidiasis, tinea), or bacterial infections (e.g., impetigo), as well as in patients with hypersensitivity to any component of the formulation.
Although generally considered safe, even for long-term use in adults, there is a risk of overdose. In infants and children, it may lead to adrenal suppression. Extra caution is required in these patients, particularly in conditions such as napkin rash, where occlusion may increase absorption. Treatment duration should generally not exceed seven days.
When inflammatory lesions are infected, appropriate antimicrobial therapy should be initiated. If infection spreads, topical corticosteroids should be discontinued and systemic antimicrobial treatment should be started.
Prolonged application to the face is not recommended. In psoriasis, topical corticosteroids should be used cautiously due to risks of relapse, tolerance, and possible systemic toxicity caused by impaired skin barrier function. Careful supervision is necessary.
When applied near the eyes, care should be taken to avoid contact with the eyes, as it may lead to glaucoma.
Side Effects
If signs of hypersensitivity occur, the medication should be discontinued immediately. When applied over large areas, sufficient absorption may occur to cause temporary adrenal suppression, despite the relatively low systemic activity of Clobetasone Butyrate.
Local atrophic changes may develop with prolonged use, particularly in conditions that increase absorption such as moisture. Changes in skin pigmentation and increased hair growth have been reported with topical corticosteroids. Worsening of symptoms may also occur.
Pregnancy & Lactation
There is insufficient evidence regarding the safety of this medication in human pregnancy. Topical corticosteroids have shown fetal abnormalities in animal studies, including cleft palate and intrauterine growth retardation. Therefore, there may be a small potential risk in humans.
Precautions & Warnings
When applied over large areas, on broken skin, or under occlusive dressings, sufficient absorption may occur to produce systemic effects. It should be used cautiously in patients with peptic ulcer, osteoporosis, psychoses or severe psychoneuroses, congestive heart failure, hypertension, diabetes mellitus, epilepsy, glaucoma, infectious diseases, ocular herpes simplex, chronic renal failure, uremia, or active/quiescent tuberculosis. Caution is also required in elderly patients and in prolonged use on the face.
Overdose Effects
Acute overdose is unlikely. However, in cases of chronic overuse or misuse, features of hypercorticism may develop, and treatment with topical corticosteroids should be discontinued.
