Examples of such infections include interdigital mycoses (such as athlete’s foot), paronychia associated with nail infections, fungal infections in skin folds, Candida vulvitis, Candida balanitis, pityriasis versicolor, and erythrasma.

Description

Clotrimazole is a broad-spectrum antifungal agent used to treat various skin infections caused by dermatophytes, yeasts, moulds, and certain bacteria. It is also effective against some gram-positive bacteria, especially Staphylococci, and organisms like Trichomonas. Like other imidazole derivatives, it alters the permeability of fungal cell membranes by interfering with cell wall lipids. At lower concentrations, it inhibits ergosterol synthesis, while at higher concentrations, it can directly damage the fungal cell membrane through additional mechanisms.

Pharmacology

Clotrimazole primarily acts by disrupting the permeability of fungal cell membranes. It inhibits ergosterol biosynthesis, an essential component of fungal cell membranes, preventing the formation of a functional membrane. Since ergosterol is vital for fungal cell growth, its depletion leads to dose-dependent inhibition of fungal proliferation.

In addition to reducing ergosterol through inhibition of lanosterol 14-demethylase (CYP51), clotrimazole also affects other cellular mechanisms. These include inhibition of sarcoplasmic reticulum Ca²⁺-ATPase, depletion of intracellular calcium, and blockage of calcium-dependent potassium channels and voltage-dependent calcium channels. These actions contribute to its broader pharmacological effects beyond antifungal activity.

Dosage Administration

Eye ointment: It should be applied thinly and evenly to the conjunctival sac every 4 hours daily or as advised by a physician.
