Indications

Tadalafil is indicated in:

  • Erectile Dysfunction (ED)
  • Benign Prostatic Hyperplasia (BPH)
  • Erectile Dysfunction with associated signs and symptoms of Benign Prostatic Hyperplasia
Pharmacology

Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor. Inhibition of PDE5 increases cyclic guanosine monophosphate (cGMP) levels in smooth muscle cells. Increased cGMP leads to relaxation of smooth muscles and enhanced blood flow into the corpus cavernosum, resulting in penile erection. PDE5 is also present in the smooth muscles of the prostate and bladder. Inhibition of this enzyme increases cGMP levels, causing smooth muscle relaxation in these tissues. This improves blood flow to the lower urinary tract and helps relax the bladder neck, thereby improving urinary flow and voiding symptoms.

Dosage Administration

Erectile Dysfunction: For most patients the recommended starting dose is 10 mg. The dose may be increased to 20 mg or decreased to 5 mg based on requirement. The maximum dosing frequency is once daily. Tadalafil is effective for up to 36 hours.

Benign prostatic hyperplasia: The recommended dose is 5 mg taken at the same time every day.

Combined Erectile Dysfunction and Benign prostatic hyperplasia: The recommended dose is 5 mg at the same time every day.

Interactions

May interact with Nitrates (e.g., Isosorbide, Nitroglycerin), Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids (magnesium hydroxide/aluminum hydroxide), Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, other HIV protease inhibitors, Rifampin, Carbamazepine, Phenytoin & Phenobarbital.

Contraindications
  • Use of nitrates (e.g., Nitroglycerin, Isosorbide) may increase hypotensive effects.
  • Hypersensitivity to Tadalafil.
Side Effects

Common: Headache, Dyspepsia, Back pain, Myalgia, Nasopharyngitis, Nasal congestion.
Rare: Vision color changes, sudden vision loss, hearing loss, Stevens-Johnson Syndrome, exfoliative dermatitis, angina, stroke, myocardial infarction, severe hypotension, tachycardia.

Pregnancy & Lactation

Pregnancy Category B. Use only if benefit outweighs risk. Unknown if excreted in human milk; caution in nursing mothers.

Precautions & Warnings

Use with caution in angina, renal or hepatic impairment, bleeding conditions, alcohol use, nitrates, alpha blockers, CYP3A4 inhibitors, and other PDE5 inhibitors.
