Indications

This preparation provides temporary relief of symptoms due to the common cold, hay fever (allergic rhinitis), or other upper respiratory allergies. These symptoms include

  • non-productive cough due to minor throat or bronchial irritation,
  • runny nose, sneezing,
  • itching of the nose or throat,
  • itchy and watery eyes,
  • and nasal congestion.
Composition

Each 5 ml syrup contains:

  • Dextromethorphan Hydrobromide BP 20 mg
  • Phenylephrine Hydrochloride BP 10 mg
  • Triprolidine Hydrochloride BP 2.5 mg
Pharmacology

This preparation is a combination of an antitussive, decongestant, and antihistamine agent. Dextromethorphan is a safe, effective, non-narcotic antitussive that acts centrally on the cough center in the medulla. Although structurally related to morphine, it has no analgesic or habit-forming properties and generally has little sedative effect. Phenylephrine is a decongestant that constricts blood vessels in the nasal passages. It is used for temporary relief of nasal, sinus, and ear congestion caused by the common cold, flu, allergies, sinusitis, and bronchitis. It reduces swelling in the nose and ears, thereby improving breathing and relieving discomfort. Triprolidine is an antihistamine used to relieve symptoms of allergy, hay fever, and the common cold, including rash, watery eyes, itchy eyes/nose/throat/skin, cough, runny nose, and sneezing. It is a potent competitive H1-receptor antagonist of the pyrrolidine class with mild central nervous system depressant effects that may cause drowsiness. It also helps reduce body fluid secretions by antagonizing acetylcholine, thereby relieving symptoms such as watery eyes and runny nose.

Dosage Administration

Adults & Children over 12 years: 1 teaspoonful (5 ml) 4 times a day every 4 hours interval.

6–12 years: 1/2 teaspoonful 4 times a day every 4 hours interval.

Use in children and adolescents: A physician's advice should be obtained before administering this combination to children less than 6 years.

Interactions

Taking certain MAO inhibitors with this medication may cause serious or potentially fatal drug interactions. Drugs such as isocarboxazid, methylene blue, moclobemide, phenelzine, procarbazine, rasagiline, selegiline, and tranylcypromine should be avoided during treatment. Most MAO inhibitors should also not be taken within two weeks before starting this medication.

Contraindications

It should be avoided in patients with liver disease or asthma. It is contraindicated in patients receiving monoamine oxidase inhibitors or within 2 weeks of stopping such therapy.

Side Effects

It may cause drowsiness and constipation. Other possible effects include gastrointestinal discomfort. There is no clear evidence of morphine-type physical dependence.

Pregnancy & Lactation

There are no specific data on the use of this combination during pregnancy and lactation.

Precautions & Warnings

This medicine may cause drowsiness. Patients should not drive or operate machinery if affected. Alcohol should be avoided during treatment. Use with caution in epilepsy, prostatic hypertrophy, glaucoma, liver disease, hypertension, heart disease, diabetes, and hyperthyroidism. If symptoms do not improve within one week or are accompanied by high fever, consult a physician before continuing use.

Overdose Effects

In cases of overdose, hospital admission is strongly recommended. Overdose may cause respiratory depression, paranoid psychosis, delusions, hallucinations, and convulsions.
