Indications

Lidocaine Spray is a topical anesthetic used for:

  • Preventing pain during minor surgical procedures of the ear, nose, and throat
  • Reducing pain and discomfort during dental procedures (e.g., before injections)
  • Suppressing cough reflex during general anesthesia
  • Minimizing pain during the final stages of childbirth, especially before episiotomy or suturing of the perineum
Pharmacology

Lidocaine works by blocking the influx of sodium ions through voltage-gated sodium channels in neuronal cell membranes. This prevents the generation of action potentials and interrupts nerve signal transmission. The drug binds more effectively to sodium channels in their activated state, resulting in a faster onset of nerve blockade, particularly in rapidly firing neurons. This mechanism is known as state-dependent blockade.

Dosage Administration
  • Each spray delivers 10 mg of Lidocaine. The maximum dose is 20 sprays (200 mg).
  • In dentistry, the normal dose is 1–5 sprays. Two sprays per quarter of the mouth is recommended, with a maximum of 3 sprays per quarter of the mouth over 30 minutes.
  • In sinus procedures 3 sprays are used.
  • In procedures of the throat and windpipe, up to 20 sprays may be necessary.
  • Up to 20 sprays may be necessary in child birth (cesarian procedure).
  • Lower doses are used for children aged 3–12 years. Lidocaine 10% Spray is not recommended for children under 3 years.
Contraindications

Lidocaine Spray should not be used in patients with myasthenia gravis, epilepsy, significant cardiac disorders, or severe liver impairment.

Side Effects

Possible side effects include skin rash or irritation, drowsiness, wheezing or breathing difficulty, chest pain, severe itching, excessive sweating, and numbness.

Pregnancy & Lactation

There are no adequate studies available. It may be used during pregnancy if deemed necessary by a physician. A small amount of lidocaine may pass into breast milk; however, it is generally not sufficient to cause harm to the infant.

Precautions & Warnings

Patients should avoid driving or operating machinery after using Lidocaine 10% Spray, as it may cause drowsiness and slow reflexes. Food and drink should be avoided for at least one hour after application in the mouth or throat to prevent choking or aspiration. Avoid spraying near the eyes. If accidental exposure occurs, rinse immediately with plenty of water for at least 15 minutes and seek medical advice.

Overdose Effects

Early signs of overdose may include lightheadedness, dizziness, drowsiness, and blurred vision. In severe cases, symptoms may progress to tremors, seizures, or loss of consciousness.
