Indications

Indapamide tablets are indicated for the management of essential hypertension. They remain effective in patients with impaired renal function, although the diuretic effect may be diminished. Indapamide is also used to treat fluid and sodium retention associated with congestive heart failure. The usual recommended dose is one tablet daily, preferably taken in the morning. In more severe cases, it may be used in combination with other antihypertensive agents.

Pharmacology

Indapamide is an orally administered antihypertensive agent. Its exact mechanism of action is not fully understood, but both vascular and renal effects contribute to its blood pressure–lowering properties. It may help reduce cardiac hypertrophy, decrease arterial wall thickening, prevent fibronectin accumulation in coronary vessels, and promote vasodilation through free radical scavenging. It also interacts with renal carbonic anhydrase. Indapamide acts primarily at the proximal segment of the distal tubule in the kidney. It promotes the excretion of sodium and chloride in roughly equal amounts, with minimal effect on potassium or uric acid excretion. A noticeable increase in urine volume is typically observed only at doses above 1.5 mg/day (sustained release). Plasma sodium levels generally remain unchanged, although hypokalemia has been reported in some patients.

Absorption: Indapamide is rapidly and completely absorbed from the gastrointestinal tract. Food may slightly increase absorption, but this is not clinically significant. Peak serum levels occur حوالي 12 hours after a single dose, and repeated dosing stabilizes serum concentration.

Distribution: The drug is widely distributed in the body and binds extensively to red blood cells and plasma proteins. It has a half-life of about 14–24 hours and reaches steady-state levels within 7 days without accumulation.

Metabolism: Indapamide is extensively metabolized in the liver, with only a small portion excreted unchanged in urine.

Elimination: The drug is primarily eliminated through urine (about 70%) and feces (about 22%) as inactive metabolites.

Dosage Administration

One tablet daily preferably in the morning. In more sever case Indapamide can be combine with other categories of anti-hypertensive agent. The safety and effectiveness in pediatric patients have not been established.

Interactions

Other Antihypertensive Agents: Indapamide may add to or potentiate the action of other antihypertensive drugs. In limited controlled trials comparing Indapamide combined with other antihypertensive drugs versus the other drugs alone, there was no notable change in the nature or frequency of adverse reactions associated with combined therapy.

Norepinephrine: Indapamide, like thiazides, may decrease arterial responsiveness to norepinephrine, but this diminution is not sufficient to preclude the effectiveness of the pressor agent for therapeutic use.

Contraindications

This drug must not be taken in the following conditions: Known allergy to this drug or to sulfonamides, Renal failure, Serious liver disease, Hypokalemia (low potassium).

Side Effects

Common side effects of Indapamide may include dizziness, headache, loss of appetite, nausea, vomiting, constipation, diarrhea, and postural hypotension (a drop in blood pressure upon standing). Electrolyte disturbances may occur, such as hypochloremic alkalosis, hyponatremia, hypokalemia, and hyperuricemia. Hypersensitivity reactions have also been reported, including skin rashes, cholestatic jaundice, and blood disorders such as thrombocytopenia, leukopenia, and aplastic anemia.

Pregnancy & Lactation

Pregnancy: There are no adequate and well-controlled studies in pregnant women; therefore, Indapamide is not recommended during pregnancy. Lactation: Mothers taking Indapamide should not breastfeed.

Precautions & Warnings

The drug should be used with caution in the following situations: Disturbed water/electrolyte balance,Diabetes ,Gout, Kidney problems. Monitoring: Monitoring of potassium and uric acid serum levels is also recommended.
