Acyclovir is a potent antiviral agent that is highly effective against Herpes simplex virus (types I and II) and Varicella zoster virus, while showing low toxicity to normal human cells. Once it enters infected cells, it is converted into its active form, acyclovir triphosphate, through a process initiated by viral thymidine kinase. This active form inhibits viral DNA polymerase and interferes with viral DNA synthesis, thereby stopping the replication of the virus without affecting normal cellular functions.
Dosage Administration
The dosage for all age groups is the same. A 10 mm ribbon of the ointment should be placed inside the lower conjunctival sac five times a day at approximately four hourly intervals. Treatment should continue for at least 3 days after healing
Interactions
No clinically significant drug interactions have been reported with Acyclovir ophthalmic.
Contraindications
Acyclovir is contraindicated in patients with known hypersensitivity to acyclovir, valacyclovir, or any component of the formulation.
Side Effects
Very common side effects include superficial punctate keratopathy, which usually resolves without stopping treatment. Common reactions include mild, temporary eye irritation or stinging immediately after application and conjunctivitis. Rarely, inflammation of the eyelids (blepharitis) may occur.
Pregnancy & Lactation
Acyclovir is classified as pregnancy category B. However, there are no adequate and well-controlled studies in pregnant women. It is not known whether topical acyclovir is excreted in breast milk, so caution is advised during pregnancy and breastfeeding.
Precautions & Warnings
The recommended dose, frequency, and duration of treatment should not be exceeded. Avoid touching the tip of the tube to the eye or any surface to prevent contamination.
Overdose Effects
No harmful effects are expected even if the entire contents of the tube (90 mg Acyclovir) are accidentally swallowed.
