This sterile ophthalmic solution is indicated for lowering elevated intraocular pressure in patients with open-angle glaucoma or ocular hypertension who do not respond adequately to topical beta-blocker therapy.

The combination of Dorzolamide Hydrochloride and Timolol Maleate works synergistically to reduce intraocular pressure. Dorzolamide inhibits carbonic anhydrase II, leading to decreased formation of bicarbonate ions and reduced aqueous humor production by limiting sodium and fluid transport. Timolol, a non-selective beta-adrenergic blocker without intrinsic sympathomimetic or membrane-stabilizing activity, further lowers intraocular pressure by reducing aqueous humor formation in the ciliary body.
Dosage Administration
Instill one drop in the conjunctival sac of the affected eye(s) twice daily.
Use in children: Safety and effectiveness in children below the age of 2 years have not been established.
Use in elderly patients: No overall differences in safety and effectiveness have been observed between elderly and other adult patients.
Interactions
Concurrent use with oral calcium channel blockers, beta-blockers, antiarrhythmic agents (such as amiodarone), digitalis glycosides, parasympathomimetics, narcotics, or MAO inhibitors may enhance hypotension and bradycardia. Combined use with oral carbonic anhydrase inhibitors or additional beta-blockers should be avoided due to additive effects. Increased systemic beta-blockade may occur when used with CYP2D6 inhibitors (e.g., quinidine, SSRIs).
Contraindications
This preparation is contraindicated in individuals with known hypersensitivity to any of its components.
Side Effects
Patients may experience mild ocular irritation, burning sensation, redness of the eye, or temporary blurred vision.
Pregnancy & Lactation
Use in pregnancy: Adequate studies in pregnant women are lacking; therefore, the medication should be used only if the expected benefit justifies the potential risk to the fetus.
Use in lactation: Caution is advised when administering this eye drop to breastfeeding mothers.
Precautions & Warnings
For ophthalmic use only. Contact lenses should be removed before instillation and may be reinserted after at least 15 minutes.
