Betamethasone Sodium Phosphate
Generic MedicinePharmacology
Corticosteroids exert their effects by binding to glucocorticoid receptors, thereby suppressing pro-inflammatory pathways and enhancing anti-inflammatory responses. Betamethasone sodium phosphate has a short duration of action because it is rapidly converted to active betamethasone. Corticosteroids possess a broad therapeutic range, as therapeutic doses may exceed normal endogenous production. Patients receiving corticosteroid therapy should be advised about the potential for hypothalamic-pituitary-adrenal (HPA) axis suppression and an increased risk of infections.
Dosage Administration
Eye: 1 or 2 drops to each affected eye every 1 or 2 hours until control is achieved, then the frequency may be reduced.
Ear: 2 or 3 drops to each affected ear every 2 or 3 hours until control is achieved, then the frequency may be reduced.
Nose: 2 or 3 drops instilled into each nostril 2 or 3 times daily.
Interactions
No specific drug interaction studies have been conducted.
Side Effects
Hypersensitivity reactions may occur, presenting as irritation, burning, stinging, itching, or dermatitis. The use of topical corticosteroids may lead to corneal ulceration and increased intraocular pressure, which can result in optic nerve damage and decreased visual acuity. Prolonged or intensive use may also cause the development of posterior subcapsular cataracts. In conditions associated with thinning of the cornea or sclera, corticosteroid therapy may further weaken the globe and increase the risk of perforation. Additional adverse effects reported with ophthalmic corticosteroid use include mydriasis, ptosis, and punctate epithelial keratitis.
Pregnancy & Lactation

Precautions & Warnings
Topical corticosteroids should not be used in cases of an undiagnosed red eye. Ophthalmic corticosteroid therapy should not be repeated or extended without regular evaluation to monitor for increased intraocular pressure, cataract development, or unrecognized infections.
Overdose Effects
Prolonged and intensive topical use may result in systemic effects.
Therapeutic Class
