Almotriptan

Indications

Almotriptan is used for the treatment of acute migraine attacks, whether accompanied by aura or not. It is also approved for use in adolescents aged 12 to 17 years for migraine management.

Pharmacology

Almotriptan is a selective serotonin (5-HT1B/1D) receptor agonist. It works by binding to serotonin receptors in the cranial blood vessels, leading to constriction of these vessels and inhibition of vasoactive peptide release. This action helps relieve migraine symptoms. It has minimal effects on other blood vessels, including those in the heart and lungs.

Dosage Administration

Acute Treatment of Migraine Attacks: The recommended dose of Almotriptan in adults and adolescents age 12 to 17 years is 6.25 mg to 12.5 mg, with the 12.5 mg dose tending to be a more effective dose in adults. If the headache is relieved after the initial almotriptan malate dose but returns, the dose may be repeated after 2 hours. The maximum daily dose should not exceed 25 mg. The safety of treating an average of more than four migraines in a 30-day period has not been established.

Hepatic Impairment: The recommended starting dose of almotriptan malate in patients with hepatic impairment is 6.25 mg. The maximum daily dose should not exceed 12.5 mg over a 24-hour period

Renal Impairment: The recommended starting dose of almotriptan malate in patients with severe renal impairment is 6.25 mg. The maximum daily dose should not exceed 12.5 mg over a 24-hour period

Interactions

Use of this medication may lead to prolonged vasospasm. Serious cases of serotonin syndrome have been reported when triptans are used together with SSRIs or SNRIs.

Contraindications

Almotriptan should not be used in patients with a history or signs of ischemic heart disease such as myocardial infarction, angina, silent ischemia, or Prinzmetal’s angina. It is also contraindicated in patients with severe or uncontrolled hypertension. Concurrent use with ergotamine, its derivatives, or other 5-HT1B/1D agonists is not recommended.

Side Effects

Rare but serious heart-related events, including myocardial infarction, have been reported with almotriptan use. These events are uncommon and usually occur in patients with underlying risk factors for coronary artery disease.

Pregnancy & Lactation

This medication falls under Pregnancy Category C. There is insufficient information regarding its excretion into human breast milk.

Precautions & Warnings

This medication should not be used in patients with known hypersensitivity to the active substance or any of its components. Caution or avoidance is required in patients with severe liver impairment, a history of cerebrovascular events such as stroke (CVA) or transient ischemic attack (TIA), and those with peripheral vascular disease.
