This combination is used for the treatment of patients suffering from moderate to severe depression accompanied by moderate to severe anxiety. Symptoms that may show improvement early in the course of treatment include insomnia, feelings of guilt or worthlessness, agitation, psychological and physical anxiety, suicidal thoughts, and loss of appetite.

Amitriptyline is a tricyclic antidepressant, while chlordiazepoxide is an anxiolytic agent. Amitriptyline works by inhibiting the reuptake of norepinephrine and serotonin in the brain, thereby increasing their levels and producing antidepressant effects. Chlordiazepoxide enhances the action of gamma-aminobutyric acid (GABA) by increasing chloride ion influx through GABA receptor channels, leading to neuronal membrane hyperpolarization. This results in overall central nervous system inhibition, producing calming, sedative, and anxiolytic effects.
Dosage Administration
Optimum dosage varies with the severity of the symptoms and the response of the individual patient. When a satisfactory response is obtained, the dosage should be reduced to the smallest amount needed to maintain the remission. The larger portion of the total daily dose may be taken at bedtime. In some patients, a single dose at bedtime may be sufficient. This tablet in an initial dosage of 3 or 4 tablets daily in divided doses is satisfactory. Or directed by the physican.
Interactions
Due to the amitriptyline component, this combination may reduce or block the antihypertensive effects of guanethidine and similar drugs with comparable mechanisms of action.
Contraindications
This combination is contraindicated in patients with known hypersensitivity to benzodiazepines or tricyclic antidepressants. It should not be used together with monoamine oxidase inhibitors (MAOIs).
Side Effects
Common effects: Symptoms similar to depression such as loss of appetite, fatigue, weakness, restlessness, and lethargy may occur.
Adverse reactions: Most commonly reported effects include drowsiness, dry mouth, constipation, blurred vision, dizziness, and abdominal bloating. Less frequent effects include vivid dreams, sexual dysfunction, tremor, confusion, and nasal congestion.
Pregnancy & Lactation
The safety of this combination during pregnancy and lactation has not been established.
Precautions & Warnings
Use with caution in patients with a history of seizures. Close monitoring is required in patients with hyperthyroidism or those receiving thyroid medications. Caution should also be exercised in patients with renal or hepatic impairment. All pediatric patients receiving antidepressants should be carefully monitored for worsening symptoms, suicidal tendencies, or unusual behavioral changes.
Special Populations
Pediatric Use: Safety and effectiveness in children and adolescents have not been fully established. The decision to use this combination should consider potential risks and clinical necessity.
Geriatric Use: In elderly or debilitated patients, the lowest effective dose should be used to minimize risks such as ataxia, excessive sedation, confusion, and anticholinergic effects.
Overdose Effects
Overdose of this class of drugs may be fatal. Amitriptyline overdose may lead to serious complications such as cardiac arrhythmias, severe hypotension, seizures, and CNS depression including coma. Benzodiazepine overdose may cause drowsiness, confusion, coma, and reduced reflexes.
Abrupt discontinuation of chlordiazepoxide may lead to withdrawal symptoms such as convulsions, tremors, abdominal cramps, vomiting, and sweating.
Milder withdrawal symptoms (e.g., insomnia, mood disturbances) may occur after stopping long-term benzodiazepine use. Withdrawal symptoms such as nausea, headache, and malaise may also occur after sudden discontinuation of amitriptyline.
