Meclizine + Pyridoxine

Indications

Prevention and treatment of nausea, vomiting, dizziness, motion sickness, radiation sickness and vertigo associated with disorders of the vestibular system (such as Meniere’s syndrome, labyrinthitis and other vestibular disturbances).

Pharmacology

Meclizine is a piperazine-derivative antihistamine with antiemetic, anticholinergic and antihistaminic properties. It acts by reducing the sensitivity of the vestibular system and helps control nausea and vomiting by acting on pathways from the chemoreceptor trigger zone (CTZ) to the vomiting center (VC) and other central nervous system centers. Pyridoxine (Vitamin B6) is added to enhance the antiemetic effect and also serves as a dietary supplement.

Dosage Administration

Adult and Children 12 years of age & over:

  • Nausea and vomiting: 25-50 mg daily or as directed by a physician.
  • Motion sickness: Take an initial dose of 25-50 mg, 1 hour prior to travel. May repeat the dose every 24 hours for the duration of the journey.
  • Radiation sickness: 50 mg administered 2-12 hours prior to radiation treatment.
  • Vertigo: 25-100 mg daily in divided doses.
  • Prevention of nausea and vomiting associated with emergency contraceptive pill (ECP): 25-50 mg, 1 hour before first ECP dose; repeat if needed in 24 hours.

The safety and efficacy for use in children less than 12 years of age have not been established.

Interactions

The CNS depressant effects of Meclizine may be enhanced when taken with ethanol or other CNS depressants such as benzodiazepines, barbiturates, tricyclic antidepressants, opioid agonists, skeletal muscle relaxants, and antihistamines. Concomitant use of other anticholinergic drugs may also increase the anticholinergic effects of Meclizine. Meclizine may increase the absorption of digoxin by reducing gastrointestinal motility.

Contraindications

Meclizine Hydrochloride and Pyridoxine Hydrochloride are contraindicated in patients with known hypersensitivity to any of the ingredients.

Side Effects

Common side effects include drowsiness and dry mouth. Rarely, blurred vision has been reported.

Pregnancy & Lactation

Pregnancy Category B. Human studies have not shown evidence of fetal harm. Available data suggest Meclizine has a low risk of teratogenicity and may be considered one of the preferred options for treating nausea and vomiting during pregnancy. Safety during breastfeeding has not been established.

Precautions & Warnings

Patients should be cautioned that Meclizine Hydrochloride may reduce alertness and impair performance of tasks requiring mental focus or physical coordination, such as driving or operating machinery. Alcohol should be avoided during treatment. Due to its anticholinergic effects, caution is required in patients with asthma, glaucoma, or prostatic enlargement.

Overdose Effects

Symptoms may include extreme excitability, seizures, drowsiness, and hallucinations.
Treatment is supportive and symptomatic; dialysis may be considered if necessary.
