Mecobalamin injection is indicated in

• Treatment of peripheral neuropathies and megaloblastic anemia due to vitamin B12 deficiency.

Mecobalamin is an active coenzyme form of vitamin B12. It differs from cyanocobalamin as it contains a methyl group instead of a cyano group. It acts as a cofactor for methionine synthase, which helps convert homocysteine to methionine through methyl group transfer. It is essential for normal myelin formation, proper nerve function, red blood cell production, and DNA synthesis. Mecobalamin also supports nerve lipid synthesis, axonal regeneration, improves axonal transport, and shows neuroprotective effects.

Tablet: 0.5 mg tablet 3 times daily after meal. The dosage should be adjusted according to age of patient’s and the severity of the symptoms.

Injection:

• Peripheral Neuropathies: Adult: 1 ampoule (Mecobalamin 500 mcg) daily, administered IM or IV 3 times a week. The dose may be adjusted depending on the patient's age and symptoms.
• Megaloblastic Anemia: Adult: 1 ampoule (Mecobalamin 500 mcg) daily, administered IM or IV 3 times a week. After 2 months of medication, the dose should be reduced to a single administration of 1 ampoule at 1 to 3 month intervals for maintenance therapy.

Use in Children: Not Recommended.

The absorption of vitamin B12 from the gastrointestinal tract may be reduced by neomycin, aminosalicylic acid, histamine H2-receptor antagonists, omeprazole, and colchicine. Serum levels may also be decreased by oral contraceptives. Most of these interactions are unlikely to be clinically significant, but they should be considered when interpreting blood concentration tests.

Mecobalamin is contraindicated in patients with known hypersensitivity to Mecobalamin or any component of the formulation.

Skin reactions: rash; treatment should be discontinued if these occur.

Gastrointestinal: loss of appetite, nausea, vomiting, and diarrhea.

Central nervous system: headache.
Others:

• anaphylactoid reactions such as hypotension or dyspnea may occur and require immediate discontinuation of treatment.
• Additional effects include hot sensation,
• sweating,
• pain or induration at the injection site.

There are no sufficient data on the use of mecobalamin in pregnant or lactating women. However, vitamin B12 is excreted in breast milk, and the American Academy of Pediatrics considers it generally compatible with breastfeeding.

This product should not be used for more than one month without clear benefit. Long-term high-dose use is not recommended, especially in individuals exposed to mercury or mercury compounds in their occupation.
