Measles and Rubella

Indications

This vaccine is recommended for simultaneous protection against measles and rubella in individuals aged 8 months and older.

Description

This is a live attenuated viral vaccine used for protection against measles (rubeola) and rubella (German measles). It is a sterile lyophilized preparation containing attenuated measles virus (CAM-70 strain grown in chicken embryo fibroblast cells) and rubella virus (RA27/3 strain grown in human diploid cells). The viruses are mixed prior to lyophilization. Before reconstitution, it appears as a light yellow compact powder or crystalline mass. After reconstitution, it forms a clear pale yellow to pinkish-yellow solution.

Composition

Each vial contains a lyophilized preparation of live attenuated measles virus (Long-47 strain) grown in chicken embryo cell cultures and live attenuated rubella virus (RA27/3 strain) grown in human diploid cells. After reconstitution, each 0.5 mL dose contains ≥1000 CCID50 of measles virus and ≥1000 CCID50 of rubella virus.

Dosage Administration

Adults and Children: A single dose of 0.5 mL of the reconstituted vaccine should be administered by deep subcutaneous injection. Do not administer intravenously.

EPI schedule: After completion of 9 months, one dose of measles-rubella vaccine should be given. After completion of 15 months, one dose of measles-containing vaccine should be administered.

Contraindications

Hypersensitivity to any component of the vaccine. Individuals with acute illness, severe chronic diseases, exacerbation of chronic conditions, or fever should not be vaccinated. Contraindicated in pregnant women. Also contraindicated in patients with immune deficiency, immune dysfunction, or those receiving immunosuppressive therapy. Not recommended in individuals with encephalopathy, uncontrolled epilepsy, or other progressive neurological disorders.

Side Effects

Common side-effects:

  • Pain and tenderness at the injection site may occur within 24 hours and usually resolve within 2–3 days
  • Mild fever may occur 1–2 weeks after vaccination
  • Mild rash may appear within 12 days and usually lasts no more than 2 days

Rare side-effects:

  • Severe fever may occur; symptomatic treatment is required to prevent febrile seizures

Very rare side-effects:

  • Allergic reactions such as urticaria may occur within 72 hours; prompt treatment should be given
  • Anaphylactic shock may occur within 1 hour; emergency treatment including epinephrine is required
  • Anaphylactoid purpura may occur; corticosteroids may be required for treatment
  • Thrombocytopenic purpura
  • Adults may experience arthritis, joint pain, and swelling of large joints after vaccination
Pregnancy & Lactation

Pregnancy Category C. Pregnancy should be avoided for at least 3 months after vaccination. Use with caution in breastfeeding women.

Precautions & Warnings

Do not use the vaccine if the vial is cracked, the label is unclear, the product is expired, or if abnormal appearance (such as turbidity) is observed after reconstitution. The vaccine should be used immediately after opening. Emergency medications such as epinephrine should be available in case of severe allergic reactions. Individuals receiving the vaccine should be observed for at least 30 minutes after administration. Those receiving immunoglobulin should wait at least 3 months before vaccination to avoid interference with the immune response. Other live attenuated vaccines should be administered at least 1 month apart from this vaccine, but it may be given simultaneously with the live mumps vaccine. This is a live attenuated vaccine and is not recommended during epidemic conditions. Women of childbearing age should avoid pregnancy for at least 3 months after vaccination. Freezing of the vaccine is strictly prohibited.

Storage Conditions

Keep out of the reach and sight of children. Store at +2°C to +8°C. Maintain the same temperature during transportation. Do not freeze. Discard if frozen. Protect from light.
