This medicine is indicated for short-term treatment of acute painful and inflammatory conditions, including:

• Post-traumatic pain, inflammation, and swelling such as sprains
• Post-operative pain, inflammation, and swelling, for example after dental or orthopedic surgery
• Painful and/or inflammatory gynecological conditions such as primary dysmenorrhea or adnexitis
• Migraine attacks
• Painful disorders of the vertebral column (spine-related pain conditions)
• Non-articular rheumatism
• As an adjunct therapy in severe painful inflammatory infections of the ear, nose, or throat, such as pharyngotonsillitis and otitis

The exact mechanism of action of Diclofenac Potassium, like other NSAIDs, is not fully understood. However, it is believed to work mainly by inhibiting the cyclooxygenase enzymes (COX-1 and COX-2).

Diclofenac is a strong inhibitor of prostaglandin synthesis in laboratory (in vitro) studies, and therapeutic levels achieved in the body also show similar effects in vivo. Prostaglandins are chemicals that increase the sensitivity of pain receptors and enhance the pain-causing effects of bradykinin in animal models. They also play an important role in inflammation.

Since diclofenac reduces prostaglandin production, its anti-inflammatory and pain-relieving effects are likely due to decreased prostaglandin levels in peripheral tissues.

Adults: Following an initial loading dose of 50 mg, 25-50 mg is to be taken every eight hours if necessary. Migraine: An initial loading dose of 50 mg, then if necessary a further 25-50 mg after 2 hours. The maximum daily dose is 150 mg. The tablets should be swallowed whole with liquid, preferably before meals. Children: Children over 14 years of age: upto 75 mg daily in divided doses.

• Diclofenac may increase plasma levels of Lithium and Digoxin, enhancing their effects and toxicity
• Increased risk of bleeding may occur when used with anticoagulant drugs
• Concomitant use with Cyclosporin may cause kidney toxicity (nephrotoxicity)
• Serious toxicity has been reported when used with Methotrexate within 24 hours
• May reduce the effectiveness of diuretics
• Using with other NSAIDs or corticosteroids may increase the risk of adverse effects

Diclofenac Potassium tablets are contraindicated in patients with known hypersensitivity to Diclofenac. It should not be used in patients who experience asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs, as severe and occasionally fatal anaphylactic-like reactions have been reported in such cases. Diclofenac is also contraindicated for the treatment of perioperative pain in patients undergoing coronary artery bypass graft (CABG) surgery.

The side effects of Diclofenac are generally mild and temporary. However, if serious adverse effects occur, the medicine should be discontinued. Occasional side effects include epigastric pain and other gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal cramps, dyspepsia, flatulence, and loss of appetite. Rare side effects include gastrointestinal bleeding, peptic ulcer with or without bleeding or perforation, and bloody diarrhea.

Diclofenac should not be used during pregnancy unless there are strong medical reasons. If required, the lowest effective dose should be used. NSAIDs like diclofenac are not recommended during the last trimester of pregnancy. Small amounts of diclofenac may pass into breast milk; however, no harmful effects on the infant are expected.
