Benzoyl Peroxide

Indications

Benzoyl peroxide is indicated for the topical treatment of mild to moderate acne vulgaris. It may be used alone or in combination with other acne therapies such as topical retinoids, antibiotics, or preparations containing sulfur or salicylic acid.

Pharmacology

Acne vulgaris develops due to inflammation of the pilosebaceous unit. Contributing factors include excess sebum production, hormonal influences (particularly androgens), abnormal keratinization within hair follicles, bacterial proliferation, and inflammation. These processes lead to the formation of comedones such as whiteheads and blackheads. Benzoyl peroxide exerts its effect by releasing free radicals through cleavage of its peroxide bond, forming benzoyloxy radicals. These radicals interact with bacterial proteins, disrupting their function and leading to bacterial death. Additionally, benzoyl peroxide reduces keratin and sebum accumulation in hair follicles and promotes epithelial cell turnover, which helps in peeling of the skin and breakdown of comedones.

Dosage Administration

Shake well before using. It is recommended that therapy be initiated with Benzoyl Peroxide, washing the affected areas once a day during the first week, and twice a day thereafter as tolerated. Wet skin areas to be treated; apply Benzoyl Peroxide Wash, work to a full lather, rinse thoroughly and pat dry. Frequency of use should be adjusted to obtain the desired clinical response. Close cap tightly after use.

Children: Safety and effectiveness in children below the age of 12 have not been established.

Interactions

No significant interactions have been reported with other topical medications used alongside benzoyl peroxide. However, concurrent use with products that have strong peeling (desquamative), irritant, or drying effects should be avoided, as they may increase skin irritation.

Side Effects

Allergic contact dermatitis and skin dryness may occur during treatment. Excessive scaling, redness (erythema), or swelling (edema) may develop in some cases. If severe irritation occurs, the medication should be discontinued and appropriate treatment initiated. Cool compresses may help relieve symptoms. Once the reaction subsides, therapy may be resumed at a lower dose if not related to allergy.

Pregnancy & Lactation

Pregnancy: Category C. Adequate studies in pregnant women are not available. Benzoyl peroxide should be used during pregnancy only if clearly necessary.

Lactation: It is unknown whether benzoyl peroxide is excreted in human milk. Caution is advised when used in nursing mothers.

Precautions & Warnings

For external use only. Avoid contact with eyes, lips, eyelids, and mucous membranes. In case of accidental contact, rinse thoroughly with water. Benzoyl peroxide may bleach hair and fabrics. Avoid excessive sun exposure and use sunscreen during treatment. If accidentally ingested, seek medical help immediately.

  • Avoid contact with sensitive areas (eyes, lips)
  • May cause discoloration of hair and clothing
  • Use sunscreen regularly
  • Prefer oil-free cosmetics
  • Discontinue use if severe irritation occurs and consult a physician

Carcinogenesis & Fertility: Animal studies suggest a possible tumor-promoting effect, but the clinical significance in humans is unknown. It is not considered mutagenic and no evidence suggests impairment of fertility.

Overdose Effects

Excessive application will not improve results but may lead to severe skin irritation. In such cases, discontinue use and provide appropriate symptomatic treatment.
