Eye: Indicated for inflammatory ocular conditions such as uveitis, marginal keratitis, allergic conjunctivitis, blepharitis, and episcleritis where bacterial infection is present or likely to occur.

Ear: Used in otitis externa and other inflammatory ear conditions where bacterial infection is present or suspected.

Nose: Indicated for inflammatory nasal conditions associated with existing or suspected infection.

Betamethasone is a corticosteroid with strong anti-inflammatory effects, effective in various inflammatory conditions including those less responsive to treatment such as psoriasis. Its structural modification (16β-methyl group) enhances anti-inflammatory activity and reduces sodium and water retention effects associated with corticosteroids.

Neomycin sulfate is a broad-spectrum bactericidal antibiotic effective against many bacteria associated with infections. It penetrates bacterial cells and binds to the 30S ribosomal subunit, disrupting protein synthesis. This leads to the production of nonfunctional proteins and ultimately bacterial cell death.

Drop: Eye: 1 or 2 drops to be given into the eye every one or two hours. Ear: 2-3 drops into the ear every two or three hourly. Nose: 2 or 3 drops instilled into each nostril two or three times daily.

Ointment: It should be applied thinly and evenly to the conjunctival sac at night (If eye drops used during day) or 3-4 times daily (if ointment used alone).

This preparation is contraindicated in viral, fungal, tuberculous, or purulent infections. Ocular use is contraindicated in patients with glaucoma or when herpetic keratitis (e.g., dendritic ulcer) is suspected, as corticosteroids may worsen the condition and lead to severe visual impairment. Preparations containing neomycin should not be used in otitis externa if the tympanic membrane is perforated due to the risk of ototoxicity.

Allergic sensitization to neomycin is rare but may occur following topical ocular use. Corticosteroid-containing eye drops may increase intraocular pressure in some patients, particularly those with a family history of glaucoma. Prolonged use may lead to corneal thinning and possible perforation. Long-term use of topical corticosteroids has also been associated with the development of cataracts.

Pregnancy Category: Not classified. The FDA has not assigned a specific category. Topical corticosteroids have been shown to cause fetal abnormalities in animal studies. Although relevance to humans is uncertain, these preparations should be used during pregnancy only if necessary and should not be used extensively or for prolonged periods.

Topical corticosteroids should not be used extensively during pregnancy, either in large quantities or for prolonged periods. Due to the presence of a steroid component, this preparation should not be used in red eye conditions until an accurate diagnosis has been established.
