Indications

This medication is used for the short-term management of steroid-responsive eye conditions where preventive antibiotic therapy is also necessary, provided that fungal and viral infections have been ruled out.

Pharmacology

Dexamethasone, a corticosteroid, reduces inflammatory responses caused by various agents and may delay or slow the healing process. Because corticosteroids can weaken the body’s natural defense against infections, an accompanying antimicrobial agent may be prescribed when this effect is considered clinically important. The anti-infective component in this combination is included to act against specific susceptible organisms.

Neomycin sulphate is effective against several microorganisms, including Staphylococcus aureus, Corynebacterium diphtheriae, Streptococcus viridans, Escherichia coli, Klebsiella pneumoniae, Proteus vulgaris, Aerobacter aerogenes, and Haemophilus influenzae.

Polymyxin B sulphate is active against organisms such as Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli, Klebsiella pneumoniae, and Koch-Weeks bacillus.

When both a corticosteroid and an antimicrobial are required, using them in combination offers improved patient compliance and convenience. It also ensures proper dosing of both agents, compatibility within a single formulation, and accurate delivery and retention of the medication.

Dosage Administration

Apply one or two drops to each affected eye up to six times daily or more frequently if required. Severe infections may require one or two drops every 15-20 minutes initially, reducing the frequency of instillation gradually as the infection is controlled. Or as directed by registered physician.

No drug interactions have been identified.

Contraindications

Contraindicated in patients with hypersensitivity to any component of the formulation. It should not be used in cases of herpes simplex or other viral infections of the cornea and conjunctiva, as well as in fungal eye diseases and ocular tuberculosis.

Side Effects

The most commonly observed reactions are allergic sensitizations. Effects associated with the steroid component include increased intraocular pressure (IOP), which may lead to optic nerve damage, formation of posterior subcapsular cataracts, and delayed wound healing.

Pregnancy & Lactation

Pregnancy Category C. Animal studies have demonstrated harmful effects on the fetus, and there are no adequate, well-controlled studies in humans. However, the potential benefits may justify the use of this medication in pregnant women despite possible risks. It is not known whether the drug is excreted in breast milk; systemic absorption is minimal when administered ophthalmically.

Precautions & Warnings

Prolonged use should be avoided, as it may cause skin sensitization and promote the development of resistant organisms. Due to the risk of steroid-induced glaucoma and cataract formation, intraocular pressure and the lens should be monitored regularly during treatment.
