Mupirocin

Indications

Mupirocin ointment is indicated for the topical treatment of impetigo caused by Staphylococcus aureus and Streptococcus pyogenes. It is also effective in the treatment of folliculitis and furunculosis.

Pharmacology

Mupirocin is a naturally derived antibiotic produced by fermentation of Pseudomonas fluorescens. It exhibits activity against a broad spectrum of bacteria responsible for most skin infections, including Staphylococcus aureus (including methicillin-resistant strains, MRSA), other staphylococci, and streptococci. It is also active against certain gram-negative organisms such as Escherichia coli and Haemophilus influenzae.

Mupirocin inhibits bacterial protein synthesis by reversibly and specifically binding to bacterial isoleucyl-tRNA synthetase. Due to this unique mechanism, it shows no cross-resistance with other classes of antibiotics. At concentrations achieved by topical application, it acts as a bactericidal agent, although its minimum bactericidal concentration is generally higher than its minimum inhibitory concentration.

Dosage Administration

A small amount of Bactropen ointment should be applied to the affected area 3 times daily for up to 10 days. The safety and effectiveness of Mupirocin ointment have been established in the age range of 2 months to 16 years.

Pediatric Use: The safety and effectiveness of Mupirocin have been established in the age range of 2 months to 16  years. Use of the ointment in these age groups is supported by evidence from adequate and well-controlled studies of Mupirocin in impetigo in pediatric patients.

Interactions

No clinically significant drug interactions have been identified with mupirocin ointment.

Contraindications

Contraindicated in patients with a known hypersensitivity to mupirocin or any component of the formulation.

Side Effects

Possible side effects include burning, stinging, pain, itching, and rash. Some patients may also experience nausea, erythema, dry skin, tenderness, swelling, contact dermatitis, or increased exudate.

Pregnancy & Lactation

Animal studies have shown no evidence of fetal harm. However, due to limited clinical data in pregnant women, mupirocin should be used during pregnancy only if the potential benefit justifies the possible risk. It is not known whether mupirocin is excreted in human milk; therefore, caution is advised when used in nursing mothers.

Precautions & Warnings

Mupirocin ointment is not intended for ophthalmic or intranasal use. Prolonged use may lead to overgrowth of non-susceptible organisms, including fungi. Avoid contact with the eyes, especially when applied on the face. It should not be used in conjunction with cannulae or at central venous catheter sites. If sensitization or severe local irritation occurs, discontinue use and initiate appropriate alternative therapy. Mixing mupirocin with other topical preparations should be avoided as it may reduce antibacterial effectiveness and stability.

Overdose Effects

Limited data are available regarding overdose. In case of excessive use, symptomatic and supportive treatment should be provided with appropriate monitoring.
