Naphazoline Hydrochloride + Pheniramine Maleate

Indications

This eye drop is indicated for the symptomatic treatment of allergic conjunctivitis.

Pharmacology

This is a combined ophthalmic preparation containing Naphazoline Hydrochloride and Pheniramine Maleate. It combines the antihistaminic action of Pheniramine Maleate with the decongestant effect of Naphazoline Hydrochloride. Naphazoline is a direct-acting sympathomimetic agent that stimulates alpha-adrenergic receptors in conjunctival arterioles, producing vasoconstriction and reducing redness and congestion. Pheniramine Maleate is an alkylamine antihistamine with antimuscarinic and mild central sedative properties, used to relieve symptoms of hypersensitivity reactions, including allergic conjunctivitis.

Dosage Administration

One to two drops should be instilled into the affected eye(s) three to four times daily. Safety and effectiveness in children under 6 years of age have not been established.

Interactions

Arrhythmias may occur when this preparation is used with cardiac glycosides, tricyclic antidepressants, or quinidine. Patients taking monoamine oxidase inhibitors (MAOIs) may develop severe hypertensive reactions when exposed to sympathomimetic agents. Although not specifically reported with Naphazoline, such interaction is theoretically possible and should be considered.

  • Hypersensitivity to any ingredient of the formulation.
  • Narrow-angle glaucoma.

Pregnancy & Lactation

Pregnancy category C. There are limited or no adequate studies on topical Naphazoline or Pheniramine use in pregnancy. It should be used only if clearly necessary. Safety during lactation has not been established; therefore, use only when essential.

Precautions & Warnings

Use with caution in patients receiving MAO inhibitors due to risk of hypertensive crisis. Patients using other ophthalmic medications should consult a physician before use. Caution is advised in children, elderly patients, and those with cardiovascular disease, arrhythmia, uncontrolled hypertension, diabetes, urinary retention, or prostate enlargement. Use carefully in patients with sympathetic denervation conditions such as insulin-dependent diabetes, orthostatic hypotension, or hyperthyroidism. Also caution in pyloroduodenal obstruction or epilepsy. Prolonged or excessive use may cause rebound conjunctival congestion. Temporary blurred vision or pupil dilation may occur; patients should avoid driving or operating machinery until vision clears.

Overdose Effects

Overdose or accidental ingestion, especially in children, may lead to CNS depression, hypothermia, bradycardia, sweating, drowsiness, coma, followed by hypertension and later hypotension. Eye overdose can be managed by rinsing with warm water.

Storage Conditions

Store below 30°C, protect from light and moisture. Keep out of reach of children. Remove contact lenses before use and wait at least 15 minutes before reinsertion. Avoid touching dropper tip to any surface. Keep bottle tightly closed after use. Do not use after 4 weeks of opening.
