Ciprofloxacin + Dexamethasone

Indications

Dexamethasone is a glucocorticoid with strong anti-inflammatory and anti-allergic effects. It reduces inflammation by inhibiting prostaglandin synthesis through blocking phospholipase A2 and decreases the migration of inflammatory cells to the affected area.

Ciprofloxacin is a broad-spectrum antibiotic effective against both Gram-positive and Gram-negative bacteria. It works by inhibiting bacterial DNA gyrase, thereby preventing DNA replication and causing bacterial cell death.

Pharmacology

Eye: This combination is used for treating steroid-responsive inflammatory eye conditions where bacterial infection is present or likely. It is also useful in post-operative eye inflammation and other infection-related ocular inflammations.

Ear: It is indicated for ear infections with inflammation such as otitis externa and both acute and chronic otitis media. It may also be used after ear surgery to reduce inflammation and infection.

Dosage Administration

For Eye: 1 drop to be instilled into conjunctival sac(s) every four to six hours. During the initial 24 to 48 hours, the dosage may be increased to 1 drop every two hours.

For Ear:

  • Acute otitis media in pediatric patients with typanastomy tube: 4 drops instilled into the affected ear 2 times daily for 7 days.
  • Acute otitis externa: 4 drops instilled into the affected ear 2 times daily for 7 days.

Pediatric Use: Safety and effectiveness in pediatric patients below 1 year have not been established.

Interactions

Specific interaction studies with ophthalmic Ciprofloxacin and Dexamethasone have not been performed. However, systemic use of certain quinolones has been shown to increase plasma levels of theophylline, interfere with caffeine metabolism, enhance the effects of oral anticoagulants like warfarin, and may cause temporary increases in serum creatinine when used with cyclosporine.

Contraindications

Hypersensitivity to any component of the formulation. It is also contraindicated in herpes simplex and other viral infections, fungal infections (mycosis), glaucoma, newborn infants, and fungal diseases affecting the eye or ear.

Side Effects

Eye: The most commonly reported adverse effects are temporary burning or discomfort in the eye. Other reactions may include stinging, redness, itching, conjunctivitis or keratitis, swelling around the eye or face, foreign body sensation, light sensitivity, blurred vision, tearing, dryness, and eye pain. Rarely, dizziness may occur. Steroid-related effects may include increased intraocular pressure (IOP) with possible glaucoma, optic nerve damage, posterior subcapsular cataract formation, and delayed wound healing.

Ear: Common side effects include discomfort and ear pain. Other reactions may include irritability, dizziness, and redness.

Pregnancy & Lactation

Pregnancy: There are no adequate and well-controlled studies in pregnant women. This eye and ear drop should be used during pregnancy only if the potential benefit justifies the possible risk to the fetus.

Nursing Mothers: It is not known whether this medication is excreted in human milk. Therefore, caution should be exercised when administered to nursing mothers.

Precautions & Warnings

Fungal infection of the cornea should be considered after long-term steroid use or in persistent corneal ulcers. Prolonged use of antibiotics may lead to overgrowth of non-susceptible organisms. Long-term use of ophthalmic steroids may result in glaucoma, optic nerve damage, visual defects, and posterior subcapsular cataract formation. Intraocular pressure should be monitored regularly.

Storage Conditions

Store in a cool and dry place, protected from light. Keep out of reach of children. Shake well before each use. Use within 30 days after first opening. Close the bottle tightly after each use.
