Indications

Acute Cerebrovascular Accidents (Strokes): Ischemic stroke due to cerebral thrombosis or cerebral embolism, acute cerebrovascular disorders, hypertensive crisis, ischemic neurological deficits, completed stroke syndrome, multi-infarct dementia, cerebral arteriosclerosis, hypertensive encephalopathy, and post-stroke conditions following hemorrhagic stroke.

Senile Disorder: Used for the relief of psychosomatic symptoms in elderly patients associated with cerebral insufficiency, including forgetfulness, memory impairment, slow thinking, poor concentration, dizziness, mood instability, aphasia, sleep disturbances, and vasovegetative symptoms related to menopausal syndrome.

Visual Disorder: Used in vascular disorders affecting the choroid and retina due to arteriosclerosis, vasospasm, macular degeneration, arterial or venous thrombosis or embolism, and glaucoma secondary to these disorders.

Hearing Disorder: Used in hearing impairment of vascular or toxic (iatrogenic) origin, presbyacusis, Meniere’s disease, cochleovestibular neuritis, tinnitus, and dizziness associated with labyrinthine disorders.

Pharmacology

Vinpocetine improves cerebral metabolism by increasing glucose and oxygen utilization in the brain and enhancing tolerance to cerebral hypoxia. It shifts glucose metabolism toward the more efficient aerobic pathway while also supporting anaerobic metabolism. It increases ATP concentration and ATP/AMP ratio in the brain and elevates cerebral norepinephrine, dopamine, and serotonin levels.

Vinpocetine significantly enhances cerebral microcirculation by inhibiting platelet aggregation, reducing increased blood viscosity, and improving erythrocyte deformability. It also promotes oxygen transport into tissues by reducing the oxygen affinity of erythrocytes.

It selectively increases cerebral blood flow and the brain’s share of cardiac output while reducing cerebral vascular resistance without significantly affecting systemic circulation such as blood pressure, heart rate, cardiac output, or total peripheral resistance. It does not produce a steal phenomenon; instead, it improves blood flow mainly in injured and ischemic brain regions while maintaining circulation in intact areas. Vinpocetine further enhances blood flow in tissues already affected by hypoxia.

Dosage Administration

Tablet: 1-2 tablets thrice daily, the maintenance dose is 1 tablet thrice daily.

IM Injection: Daily dose of 20-40 mg are to be given until improvement of symptoms is reached (for not longer than 10 days) then oral treatment should be applied. If this regimen fails, infusion treatment should be started.

IV Infusion: The daily starting dose is 20 mg in slow drip infusion (2 ampoules in 500-1000 ml infusion solution). This dose can be increased to 1 mg/kg body weight during 3 to 4 days. Treatment should be continued for 10-14 days depending on the tolerance of the patients and the dose should be gradually reduced before discontinuation of treatment.

Interactions

Vinpocetine injection is chemically incompatible with heparin; therefore, both medications should not be administered in the same syringe.

Contraindications

Parenteral administration is contraindicated in patients with severe ischemic heart disease, severe rhythm disorders, and during pregnancy.

Side Effects

Transient hypotension and tachycardia may occur during treatment.

Pregnancy & Lactation

Vinpocetine is contraindicated during pregnancy and lactation.

Precautions & Warnings

During the acute phase, parenteral therapy is recommended until symptoms improve, followed by oral therapy. In chronic conditions, oral treatment should be continued.
