Acetazolamide

Indications

Acetazolamide is indicated for the treatment of open-angle glaucoma and secondary glaucoma. It is also used as an adjunct therapy in managing edema and epilepsy.

Pharmacology

Acetazolamide is a carbonic anhydrase inhibitor that blocks the enzyme responsible for the reversible conversion of carbon dioxide and carbonic acid. In the eye, this action reduces the production of aqueous humor, leading to decreased intraocular pressure, which is beneficial in glaucoma management. In the central nervous system, inhibition of this enzyme helps reduce abnormal neuronal activity, making it useful as an adjunct treatment for epilepsy.

Dosage Administration

Adults : Initially 250 mg (1 tablet) 4 times daily. Dose should be adjusted later with individual need.

Children : ½ to 2 tablets in divided doses.

In intraocular pressure: 1 tablet 6 hourly. In acute condition, dose should be started with 2 tablets followed by 1 tablet 6 hourly or as directed by the physician

Contraindications

Acetazolamide should not be used in patients with severe kidney or liver disease, or in those with low sodium or potassium levels in the blood. It is also contraindicated in patients with chronic non-congestive angle-closure glaucoma.

Side Effects

Common side effects include headache, drowsiness, dizziness, fatigue, loss of appetite, increased urination, insomnia, and gastrointestinal discomfort.

Pregnancy & Lactation

There are no sufficient and well-controlled studies on the use of Acetazolamide in pregnant women or newborns. Therefore, it should be used with caution during pregnancy and breastfeeding.
