Amenamevir exerts its antiviral effect by inhibiting the viral helicase-primase complex, which is essential for the unwinding of viral DNA and initiation of DNA synthesis during replication. This mechanism differs from that of conventional antivirals such as acyclovir and valaciclovir, which act by inhibiting viral DNA polymerase.
Dosage Administration
Herpes simplex: The usual adult dosage of Amenavir is 200 mg administered orally once daily after meal.
Recurrent herpes simplex: 1200 mg single dose after meals within 6 hours after the appearance of the initial symptoms of recurrent herpes simplex. (Only one dose is required).
Herpes zoster: The usual adult dosage of Amenavir is 400 mg administered orally once daily after meal.
Use in children: There is a lack of specific clinical studies evaluating the safety and efficacy of Amenamevir in children.
Interactions
Amenamevir shows clinically relevant interactions with certain drugs due to its metabolism via cytochrome P450 enzymes, particularly CYP3A4.
Ketoconazole: As a strong CYP3A4 inhibitor, it can significantly increase the plasma concentration of Amenamevir.
Rifampicin: Being a potent CYP3A4 inducer, it markedly reduces Amenamevir levels, potentially decreasing its therapeutic effectiveness.
Midazolam: As a CYP3A4 substrate, its plasma levels may decrease when co-administered with Amenamevir.
Warfarin: No clinically significant interaction has been observed, as Warfarin is mainly metabolized by CYP2C9.
Contraindications
Amenamevir is contraindicated in patients with:
- Severe renal impairment
- Known hypersensitivity to Amenamevir or any of its components
Side Effects
Common adverse effects include headache, nausea, diarrhea, abdominal pain, skin reactions, and increased levels of N-acetyl-β-glucosaminidase and α1-microglobulin. Serious adverse effects may include thrombocytopenia, gingival bleeding, and palpitations.
Pregnancy & Lactation
Pregnancy: Animal studies have not shown harmful effects on maternal or fetal health at certain doses. However, there is insufficient evidence regarding safety in pregnant women, so caution is advised.
Lactation: There is no adequate information available on the use of Amenamevir during breastfeeding.
Precautions & Warnings
Careful monitoring is recommended in patients with:
